I. The lessee agrees to pay to the lessor a~ the time of the acceptance of this
contract a deposit of fifteen dollars (S/5. 00) for each day reserved. The'
deposit will be forfeited if for any reason that dote is cancel/ed or changed.
The sum of $ 1.CiJ is due and payable upon the acceptance of this contract.
The total deposit paid wi 1/ be deducted from the total charge unless a portion
of such deposit has been forfeited. All charges made for the use of school
facilities shall be paid in advance of the date that such facility is to be used.
(See instructions for exceptions.)
Any lessee required to furnish a certificate of insurance (see Items 3 qnd 4) is
0150 required to pay a minimum rental fee of one daollar. ($I. 00).
2. The lessee agrees that:
a. All Fire Q}partment regulations must be strictly obsorved.
b. Smoking may be permitted in the halls with the principal's permission.
There will be no smokin9.-in rooms, ouditoriums,...m'mnosiums, etc.
c. Concessions wi II to operated onl y for the benefit ortlie school and with
the permission of the scho<JI principal.
d. The presence of at least one custodion is required v..hen school facilities
ore occupied by any group.
e. All school facilities and equipment used must be re.turned in as good
condition as whon rented. Cost of repairs must be borne by the les$ee.

A dupl icate copy of such Certifica te wi II be attached to, and made a part of, this
contract.
5. Special Provisions (date and time of use, etc.) u:;e of playground facilities and

o. This ogreement is subject to termination by the lessor Or lessee upon ten days
notice tathe respective party.
b. No improvements shoil be made on the lease property except by modification
of this contract in written form and signed by the parties hereto.
c. Under no circumstances shall the period of this contract extend for more than
one year.
d. Unforeseen questions shall be decided by lessee ond the principal of the facility
being leased.
9. The following Rate Schedule is agreed by the parties hereto to be valid, reasonable and
binding on said parties. The lessor reserves the right to adjust the contract chorges in
accordance with the Rate Schedule if the facility usage differs fron,the contract terms.
NOTE: addendum to no. 3
Specifically excluded herefrom are all personal injuries
or property damages when such are caused by the acts and/or
negligence of any employee, agent or representative of the
District School Board of pinollna County, Florida.

3. $5.00 per performance for each picce of equipment used,
(piano, P.A.Systerns, spotlights, etc.) ,
4. $5.00 per hour for custodial service
$4.00 per hour for Audio-Visual representati ve to supervise
and help with sound

Additional Audio-Visual assistance
5. For rehearsals: 1/10 of seat charge plus lighting cost plus
equi pment charge.
For afternoon use: 1/2 of seat charge plus lighting 'cost plus
equipment dl) rge plus custodial and other services if building
is not air conditioned. If the building is air conditioned, FULL
rates apply.
