SCHOOL FACILITY USE. CONTRACT
(NOTTO BE USED FOR ATHLETIC EVENTS OR SCHOOL RELATED GROUPS)
THIS CONTRACT IS MADE and entered into this 21st

between the SCHOOL BOARD OF PINELLAS COUNTY, FLORIDA, hereinafter called Lessor;
and the Clearwater Parks and Recreation Department
of the
City of Clearwater
, State of Florida, hereinafter called Lessee,
for use of " Dunedin Jr. High School

I.~ The lessee agrees to pay to the lessor at the time of the acceptance of this
contract a deposit of fifteen dollars ($15.00) for each day reserved. The
deposit will be forfeited if for any reason that date is cancelled or changed.
The sum of $ N/A is due and payable upon the acceptance of this contract.
The total deposit paid will be deducted from the total charge unless a portion
of such deposit has been forfeited. All charges made for the use of school
facilities shall be paid in advance of the date that such facility is to be used.
(See instructions for exceptions.)
Any lessee required to furnish a certificate of insurance (see Items 3 qnd 4) is
also required to pay a minimum rental fee of one daollar. ($1. 00).
2. The lessee agrees that:
a. All Fire ~partment regulations must be strictly observed.
b. Smoking may be permitted in the halls with the principal's permission.
There will ,be no smoking in rooms, auditoriums, g mnasiums, etc.
c. Concessions wi e operated on y for t e enefit of t e sc 00 and with
the permission of the school principal.
d. The presence of at least one custodian is required when school facilities
are occupied by any group.
e. All school facilities and equipment used must be returned in as good
condition as when rented. Cost of repairs must be borne by the lessee.
f. Space will not be available if use interferes in any way with the school
program.

3. The lessee agrees to indemnify and save and hold hormless the lessor and to acc8pt
011 lia~ility arising out of, or resultinCJ from, any occident, illness, sickness, or
other injuries including death at ony timL' (:c:.:cLJrrinCj to persons (0t-h'2r than c,nvdoyees
of the lessor, while acting inihcir capuci1ics as cmployees) on school premise', unellor
property damage to schroi premises (bui Iding, content, equipment) and to proi)(:riy of
others during time of rental or rehearsals. The lessee agrees furlher not to incur any
debts which may be required or requested to be borne by lessor.
4. The lessee shall furnish to the lessor a Certificate of I nsurance form as evidence that
the minimum amount of insurance required by this con1ract is in foci carried.
Limits of liabi lity for th is lease shall be as indicated on th,~ Certificate
of Insurance attached for bodi I y injury and property damages.
The Certificate of Insuran.ee wi II be forwarded to the Risk Mana~lement DepclIlrnent of lhe
School Board of Pinellas County, Florida, prior to occupancy and use of such focil ity.
A dupl icate copy of such Certifica te wi II be attached to, and made a part of, this
contract.
5. Special Provisions (date and time of use, etc.) Monday and Thursday evenings
from 6:30 p.m. to 10:30 p.m. - Tuesday evenings, if needed
a. This agreement is sub jeet to termination by the lessor or lessee upon ten days
notice to the respective party.
b. No improvements shai I be made on the lease property except by modification
of this contract in written form and signed by the parties hereto.
c. Under no circumstances shall the period of this contract extend for more than
one year.
d. Unforeseen questions shall be decided by lessee and the principal of the faci I ity
being leased.
6. The following Rate Schedule is agreed by the parties hereto to be valid, reasonable and
binding on said parties. The lessor reserves the right to adjust the contract charges in
accordance with the Rate Schedule if the facility usage differs frornthe contract terms.

RATE SCHEDULE
The following schedule is to be used to determinl..! chCi"ges for the use of school facilities:
I. Number of seats in room to be used is multi plied by:

5. For rehearsals: 1/10 of seat charge plus lighting cost plus
equipment charge.
For afternoon use: 1/2 of seat charge plus lighting cost plus
equipment dorge plus custodial and other services if building
is not air conditioned. If the building is air conditioned, FULL
rates appl y.
