' An architect representing Fairwinds is requesting approval of a site plan
amendment 1 to add a total of 708 square feet of floor area
(for enlargement of the proposed dining room, group therapy room and the
lobby/corridor). The additions will meet minimum setback requirements. The
proposed development will not be appreciably affected by the change.
The plan amendment has been reviewed by members of the Development Review
Committee. Those committee members who have no objection to the amendment
have been asked to initial the approval list below.

Enclosed find ].5 copies of a revised final site plan showing
as-bui.].t conditions. 'T'he changes occurred primarily because
of survey errors in tree locations and location of the existing
ditch. The changes are as follows:
1. Tlye configguration of the retention basins were changed
somewhat in shape because of tree conflicts and because the
cli.tch is further north than was shown on the survey.
2. The maintenance building was removed.
3. The pump house was to be removed. It now stays.
4. Because of the shifting of the retention basins and correct-
ed Location of trees it was necessary to revise the config-
~uration of the parking spaces ., The number of spaces remains
as before.

DATE: March 15, 1988
The Development Review Committee at its meeting of January 28, 1988
approved the above described final site plan subject to the following
conditions:
1. A copy of DER and SWFWMD applications for permit must be provided to
the Engineering Department prior to site. plan certification. (The
Public Works Department finds the submittal to be acceptable.)
2. The plan must be modified to indicate 10' drainage and utility easement
on the south .property line. (The Public Works Department finds the
revised plan to be acceptable.)
~3. All easements on this site must be shown as drainage and utility ease-
ments. (The Public Works Department finds the revised plan to be
acceptable.)
4. A letter of no objection from the owner of the property to the south of
the site to work on this property must be provided to the Engineering
Department or the plan must be modified by removing all proposed
construction or encroachment from this property, prior to certification
of the site plan. (The Public Works Department finds the developer's
proposal to be acceptable.)
5. Easements must be located so as to not have encroachments. (The Public
Works Department finds the revised plan to be acceptable.)
6. The sidewalk on Ft. Harrison must be shown or a note must be on the
plans indicating that sidewalk will be constructed. (Noted on the
revised site plan.)
7. Two (2) handicap parking spaces must be indicated on the plan. (Have
been indicated on the plan.)
8. Signs :end fencing are subject to separate permitting and should be
indicated as such on the plan.
9. Cross hatching or shading of interior parking lot landscaping should be
provided on the plan. (The Urban Forester has determined the proposed
interior landscaping is in excess of the 6y requirement, and that
shading is not necessary as it would add confusion to this detailed
plan.)

residents. (The project data has been amended to show the correct
number of 28 residents.)
11. Recreation impact fees must be paid before issuance of the building
permit for the seven (7) additional units.
12. The required si,amese connection must be shown on the same side as the
fire hydrants. (Subject to approval by the Fire Marshal prior to the
issuance of a building permit.)
13. An ingress and egress easement and a 10' utility easement over gas and
water mains up to~and including meters and hydrants must be provided
prior to the issuance of a building permit.
14. The dumpster must be in visually screened enclosure with a minimum 12'
clear opening for access. (Indicated on therevised site plan.)
15. An easement must be provided by the developer for the 2" natural gas
main currently located on the site to be relocated at City expense.
(Subject to approval by the Utilities Department.)
- 16. The requisite initial building permit must be procured within six (6)
months from the date of certification of the final plan and all
certificates of occupancy must be obtained within three years of the
date of certification of the final plan.

DATE: June 13, 1989
This site plan was originally certified on March 22, 1988 subject to
conditions. On May 16, 1989 the Planning and Zoning Board approved a
conditional use to increase the capacity from 28 to 36 adults (beds) subject
to: (a) There be no detoxification or lock up carried out in the facility:
(b) That a corrected site plan be submitted within 30 days showing revisions
to capacity and parking requirements. The site plan calculations have been
revised accordingly.
I am approving the modified plan as a simple adjustment subject to the
conditions on the March 22, 1988 certified plan.
Please certify the attached plans subject to the conditions of approval on the
site plan originally certified on March 22, 1988 and distribute.

Enclosed find 15 copies of a revised final site plan showing
as-built conditions. The changes occurred primarily because
of survey errors in tree locations and location of the existing
ditch. The changes are as follows:
1. The configguration of the retention basins were changed
somewhat in shape because of tree conflicts and because the
-ditch is furtl~rer north than was shown on the survey.

` An architect representing Fairwinds is requesting approval of a site plan
amendment for the hospital to add a total of 708 square feet of floor area
(for enlargement of the proposed dining room, group therapy room and the
lobby/corridor). The additions will meet minimum setback requirements. The
proposed development will not be appreciably affected by the change.
The plan amendment has been reviewed by members of the Development Review
Committee. Those committee members who have no objection to the amendment.

The Development Review Committee at its meeting of January 28, 1988
approved the above described final site plan subject to the following
conditions:
1. A copy of DER and SWFWMD applications for permit must be provided to
• the Engineering Department prior to site. plan certification. (The
Public. Works Department finds the submittal to be acceptable.)
2. The plan must be modified to indicate 10' drainage and utility easement
on the south .property line. (The Public Works Department finds the
revised plan to be acceptable.)
3. All easements on this site must be shown as drainage and utility ease-
ments. (The Public Works Department finds the revised plan to be
acceptable.)
4. A letter of no objection from the owner of the property to the south of
the site to work on this property must be provided to the Engineering
Department or the plan must be modified by removing all proposed
construction or encroachment from this .property, prior to certification
of the site plan. (The Public Works Department finds the developer's
proposal to be acceptable.)
5. Easements must be located so as to not have encroachments. (The Public
Works Department finds the revised plan to be acceptable.)
6. The sidewalk on Ft. Harrison must be shown or a note must be on the
plans indicating that sidewalk will be constructed. (Noted on the
revised site plan.)
7. Two (2) handicap parking spaces must be indicated on the plan. (Have
been indicated on the plan.)
8. Signs and fencing are sub3ect to separate permitting and should be
indicated as such on the plan.
9. Cross hatching or shading of interior parking lot landscaping should be
provid~.d on the plan. (The Urban Forester has determined the proposed
interior landscaping is in excess of the 63~ requirement, and that
shading is not necessary as it would add confusion to this detailed
plan.)

residents. (The project data has been amended to show the correct
number of 28 residents.)
11. Recreation impact fees must be paid before issuance of the building
permit for the seven (7) additional units.
12. The required s~amese connection must be shown on the same side as the
fire hydrants. (Subject to approval by the Fire Marshal prior to the
issuance of a building permit.)
13. An ingress and egress easement and a 10' utility easement over gas and
water mains up to and including meters and hydrants must be provided
prior to the issuance of a building permit.
14. The dumpster must be in visually screened enclosure with a minimum 12'
clear opening for access. (Indicated on the xevised site plan.)
15. An easement must be provided by the developer for the 2" natural gas
main currently located on the site to be relocated at Cit,,y expense.
(Subject to approval by the Utilities Department.)
16. The requisite initial building permit must be procured within six (6)
months from the date of certification of the final plan and all
certificates of occupancy must be obtained within three years of the
date of certification of the final plan.
