Dear Mr. Mahan:
Enclosed for your use are 12 sets of the revised Final Site Plan and a $150.00 check for the review fee for the project
referenced above. The site plan was revised to accommodate a wider driveway required on Pinellas Street.
Specifically, the plan was revised as follows:
1. The paved vehicular use area was increased from 5,688 s.f. to 6,024 s.f.
2. The open space for the site was reduced from 19,749 s.f. to 19,413 s.f.
3. The width of the driveway was increased to 40.4 feet to accommodate the truck traffic expected to utilize
the entrance.
4. The parking lot interior landscaping was reduced from 445 s.f. to 361 s.f.
Due to the minor effect of the above changes, the proposed revisions can be considered a minor site plan
amendment.

This memo is in reference to a letter from Keith Appenzeller of King Engineering to Terr Jennings
dated April 12, 1995 (attached).
Since then Bob Maran has reviewed the initial request for the credits on impact fees to the several
buildings that were demolished by Morton Plant in the last year. For our files, Bob has documented
the demolition permits applied for by Morton Plarit with Roberta and Bobbie in Central Permitting.
Bob has obtained account numbers from Joyce Geisel in Utility Accounts to verify meter sizes.
The original list of addreses and meter sizes provided by King Engineering has been corrected based
on information provided by Central Permitting and Utility Accounts. King Engineering concurrs with
Bob's records and on July 7, 1995, sent h;m a revised list of addresses and meter sizes (attached).
To the best of our knowledge, this information is, accurate.
The Powell Cancer Care Center has paid all water and sewer fees for new meters to tl~e building
even though they had demolished eight (81 buildings. It is appropriate to treat Morton Plant
Hospital as a Campus and to provide these credits to the two remaining buildings (Heart and
Vascular, 303 Pinellas Street, and Central UtilityPlant, 1233 So. Ft. Harrison Avenue).
The sum total of the credits are as follows:

The Development Review Committee at its meeting of December 8,1994 approved the above described site plan
subject to the following conditions with one condition added by the Director of Central Permitting on the
elevations (appearances) of the buildings:
1. Prior to issuance of a building permit, a landscape plan shall be submitted to Environmental
Management. .
2. Prior to the issuance of a building permit, a clearing and grubbing permit and a tree removal
permit {or a no tree verification form) are required.
3. Prior to certification, a copy of the SWFWIVID permit application and calculations shall be
provided to Environmental Management.
4. Prior to issuance of a building permit, a copy of the approved SWFWMD permit shall be
submitted to Environmental Management.
5. Prior to certification, the total paved vehicular use area and total interior landscape area
calculations shall be expressed in square feet and as a percentage of the overall site in the site
plan data block.
6. Signs and fencing/walls are subject to separate review and permitting processes.
7. The requisite building permits must be procured within one (1) year from the date
of certification of the final site plan and all requisite certificates of occupancy must be obtained
within three (3) years of the date of certification of the site plan.
8. The dates and conditions for approval by the Planning and Zoning Board for conditional use
approval and the Development Code Adjustment Board for variance approval shall be provided
on the preliminary plat/ site plan prior to certification.
9. All elevations of the building will be generally similar in appearance..
10.Show Fire Hydrant Assembly on Fort Harrison on Site Plan.
11.Sidewalks are required adjacent to all street rights of way. 5'on Ft. Harrison, 4' on Hamlet
Drive.

ITEM #6 Morton Plant Central Utility Plant (PSP 94-23); Located on South Ft. Harrison Avenue
between Pinellas Street and Grand Central Avenue. This is continued from the canceled
November 23rd DRC Meeting. The applicant proposes to construct a 18,121 sq. ft. Utility
Plant to provide energy for the physical plant of the hospital. The building is comprised of
an 5,016 sq. ft. engineering area; a 3,200 sq. ft. generator area; a 4,160 sq. ft. chillers area;
. and a 5,760 sq. ft. boiler area. Points of access are four: three on Grand Central Avenue
and one on Pinellas Street. The perimeter landscaping and associated parking is provided.
MOTION:
Lou Hilton moved to approve the preliminary site plan subject to the following conditions:
Pertinent comments from the Department representatives are incorporated into the motion listed below.
Central Permitting
1. Signs and fencing/walls are subject to separate review and permitting processes.
2. The requisite building permits must be procured within one (1) year from the date of certification of
the final site plan and all requisite certificates of occupancy must be obtained within three (3) years
of the date of certification of the site plan.
3. The dates and conditions for approval by the Planning and Zoning Board for conditional use
approval and the Development Code Adjustment Board for variance approval shall be provided on
the preliminary plat/site plan prior to certification.
Fire Department
1. Show FIRE HYDRANT ASSEMBLY on Fort Harrison on site plan.
Traffic Engineering
1. Sidewalks are required adjacent to all street rights-of-.way. 5' on Ft. Harrison, 4' on Hamlet Drive.

The applicant has submitted additional information on noise generation, addressing specific Commission
concerns. The noise report indicates noise levels affecting nearby residential areas to be less than 45
decibels, an insignificant level. Staff visited the central utility plant for the Orlando Regional Medical
Center (ORMC), which is similar. in size and equipment to that proposed for the Morton Plant Hospital.
Staff's observations of this facility indicate that the level of noise and vibration transmission. can be
adequately attenuated with appropriate acoustic design. The hospital's consultants stated that the
proposed Morton Plant Central Utility Plant design incorporates more sound attenuating devices than the
Orlando facility.
The appearance of the proposed facility was an issue of importance to the City Commission. The
revised elevation drawings indicate a significantly less "industrial" building appearance from the first
submittal.

CONCURRENCY: A traffic impact study in conjunction with the Hospital Master Plan was submitted
and approved by the City Traffic Engineer.

3. A Unity of Title shall be recorded prior to certification of the site plan.
4. The minimum turning radius for designated drives and fire lanes shall be 42 ft.
5. Afire hydrant assembly shall be installed not more than 300 ft. from the structure.
6. If structure requires or has installed an automatic fire sprinkler system, afire department
connection shall be installed a minimum of 15 ft. from the structure, adjacent to a paved drive and
within 30 ft. of a fire hydrant assembly.
7. Prior to certification, the Dumpster Requirement Form must be completed and returned to the City
Sanitation Department.
8. Prior to the issuance of a certificate of occupancy, the dumpster/compactor must be visually
screened, and gates must be provided with a minimum 12-foot clear opening to the front for
access by Sanitation trucks.
9. Prior to certification, a copy of the SWFWMD permit application is to be provided to Engineering
Site Plan Review.
