SUBJECT: Simple Adjustment to Certified Site Plan for Clearwater ~Iealth Care Center (PSP 86-24)

The applicant has submitted a request for a Simple Adjustment for the Clearwater Health Care Center Site Plan.
The change to the site plan includes the construction of a 93 square foot gazebo. The most recent site plan was
certified on July 21, 1986 and the previous conditions are carried forward to read as follows:
1. The minimum setback from the south property line must be 25 feet (indicated on plan received
7-11-86).
2. The one-way drive be changed in order for traffic to flow east to west (indicated on plan received
7-11-86 approved by Keith Crawford, Traffic Engineering on 7-17-86).
3. A note be provided on the plan that trees 4 inches in diameter at breast height will be preserved
on the site (noted on plan received on 7-11-86).
4. A note be provided on the plan that the dumpster will be visually screened with a 12 foot wide
clear opening (indicated on plan received 7=11-86).
5. Landscaping must be provided along the south property line (Oak Creek Subdivision) in such an
intensity that a person standing on the ground will not be able to see the nursing home building
(noted on plan received 7-11-86).
6. Required open space fees be paid prior to certification of the final site plan (paid on 7-18-86 to
Elizabeth Haeseker, Assistant City Manager).
7. The right-of--way from McMullen Booth, Landmark Drive,and Curlew Road shall be dedicated to
the city and an agreement shall be prepared which guarantees the dedication ofright-of--way within
six months of certification of the final site plan and preliminary plat subject to all building
inspectio~-s ceasing ifright-of--way has not. been dedicated (Elizabeth Haeseker received agreement
in letter form 7-18-86, recordable form to follow).
8. The requisite initial building permifs be issued within six months from the date of certification and
all requisite certificates of occupancy be issued within two years from the date of issuance of the
initial building permit.
This amendment to the certified site plan is approved as a simple adjustment to the Certified Site Plan for
Clearwater Health Care Center. Please certify this memo and distribute to those listed below.
