The Development Review Committee al its meeting of January 5, 1995 approved the above described site plan/
amendment subject to the following conditions:
1. Signs and fencing/ walls are subject to separate review and permitting processes.
2. The requisite building permits must be procured within one (1) year frcxn the date of
certif cation of the final site plan and all requisite certificates of occupancy must be obtained
within three (3) years of the date of certifcation of the site plans.
3. The minimum turning radius for designated drives and fire lanes shall be 42 feet.
4. Sewer impact fee is Co be paid prior to certification.
5. Site plan shall indicate handicapped spaces.
6. Prior to issuance of a building permit, a landscape plan shall be submitted to Environmental
Management.
7. Prior to the issuance of a building permit, a clearing and grubbing permit and a tree removal
permit(or a no tree verification form) are required.
8. Prior to certification, a copy of the SWF'WMD permit application and calculations shall be
provided to Environmental Management.
9. Prior to issuance of a building permit, a copy of the approved SWFWMD permit shall be
submitted to Environmental Management.
As conditions 3-9 inclusively above have been met, please certify the attached plans subject to conditions 1 &
2 listed above and distribute.

The meeting was called to order at 9:10 a,m. by Chairman Shuford in the DRC Conference
Room B.
MOTION: Lou Hilton moved to approve the minutes of the December 8, 1994 meeting as
submitted. The motion was seconded by Bob Maran and passed unanimously.
ITEM #1 Palm Garden of Clearwater; Located at tl~e southwest corner of McMullen-Booth
Road and Curle~~,~ Road. The applicant proposes to construct a 5,200 sq. ft. Therapy Addition to
the existing Palm Garden facility. The Engineer is requesting comments from staff to facilitate
this improvement.
Pertinent comments from the Department representatives are incorporated into the motion listed
below.

similar project at this location on June 16, 1994. Tlie applicant has modified the initial site plan
by relocating the proposed t~vo story 120 bed nursing }iome facility and a sales center with
associated required parking located away from the ] 00 year floodplain, placing t}~e facility at the
western portion of the existing Bayview Gardens property. The applicant proposes~to expand the
existing Ba~~~iew Gardens Continuing Care Retirement Community by constructing a 49,670 sq.
ft., two story, 120 bed nursing facility wit}i associated parking and storrnwater management. All
property owned by the applicant is no~~v s}iown on the site plan.
MOTION: Lou Hilton moved to continue this item to January l9, 1995. Tire motion was duly
