SUBJECT: Certification of Final Site Plan for Beth Shalom Congregation located at the southwest comer of
Ham Blvd. and Belcher Rd.

The Development Review Committee at its meeting of Feb. 28, 1991 approved the above described site plan
subject to the following conditions:
1) Prior to certification, the location of the fire hydrants shall be resolved with the Fire Marshal. '
2) Prior to certification, the applicant shall meet with the Department of Environmental Management to
determine a clearly delineated landscape plan.
3) The applicant shall locate the abandoned well and contact the Department of Public Works/Utilities and
identify the drainage and utility easement shown on plan.
4) Prior to the issuance of a building permit, easements must be located and 5' drainage and utility sidewalk
easement to be dedicated to the City.
5) The applicant shall show on the plan the driveways on Claiborne to be removed, the driveway aprons to
removed and replaced with sod, and show a continuous sidewalk along Claiborne upon completion of each
phase.
6) Prior to the issuance of a Certificate of Occupancy, the applicant -shall coordinate with the Department
of Public Works/Solid Waste Division for the conversion form dumpsters to 90 gallon black garbage cans.
7) The applicant shall. contact Parks & Recreation for open space assessment fee, which shall be paid prior
to certification.
8) The applicant must show the landscape areas on the existing plan.
9) Requisite initial building permit shall be procured within one year from the date of the certification and
all requisite Certificates of Occupancy shall be obtained within three years from the date of certification.
As conditions #1,2,5,7,and 8 have been met, please certify the attached plans/plats subject to conditions #3,4,6,
and 9 listed above and distribute.
