NORTHWOOD ESTATES HOMEOWNERS ASSOCIATION INC / ADOPT-A-PARK PERMIT EXHIBIT C5 PERMIT Permission and authority are hereby given and granted by the City of Clearwater, c/o Parks and Recreation Department, P.O. Box 4748, Clearwater, Florida 33758-4748, herein referred to as City, to Northwood Estates Homeowners Association, Inc., a Florida Not For Profit Corporation, hereinafter referred to as "Permit Holder", whose address is P.O. Box 13225, Clearwater, Florida 33761, for the installation of a low level neighborhood identification sign to read "Northwood Estates A DEED RESTRICTED COMMUNITY" which will be located in the first median on Eastland Avenue east of Bumtfork Drive. This permit shall be identified as EHIBITS C5, C6, C7, and C8 to ADOPT-A-PARK PERMIT Executed and agreed upon by both parties February 26,2001, and shall be attached hereto and made a part thereof. This permission and authority is subject to the following terms and conditions: 1. Permit Holder prior to beginning work on the installation of low level neighborhood identification sign shall apply for and obtain sign permit from the City Planning and Development Department. Permit to be attached to and become a part thereof as EXHIBIT C6. The City will review sign specifications as it relates to the design and type of sign, the impact on utilities, considerations of site distance problems, etc. Specifications for the size and installation of the sign shall be included with permit application as EXHIBIT C7. Aerial photograph indicating exact location of median shall be attached and included as part of permit and be known as EXHIBIT C8. 2. Permit Holder shall purchase and install the sign, which will be constructed as a single faced, sandblasted identity sign. Panel to be approximately 42" wide, mounted on a decorative 4" x 6" beveled edged posts and similar to drawing shown on EXHIBIT C7 attached to this permit Overall height of sign and post shall not exceed 18". 3. Permit Holder assumes all responsibility of maintaining the neighborhood identification sign on a regular basis. 4. The City, at such time as the Permit Holder may discontinue maintenance of the sign Retains the right to remove said sign. The Permit Holder (Northwood Estates Homeowners Association, Inc.) and its volunteers shall save and hold harmless the City of Clearwater for any injuries or property damage that may result to the Association volunteers during the performance of installation or maintenance tasks relating to the maintenance of the sign. Neighborhood Identification Sign and to any other third party relating to the placement, installation and The insurance requirements of Section 8, of the Adopt A Park Permit dated February 26, 2001, shall be amended to include coverage for the sign in addition to landscaping and landscaping materials. and shall not be assignable by the Permit Holder. This permit may be revoked at any time by the City Manager of the City of Clearwater, Florida By execution and acceptance of this permit, the Permit Holder and its duly elected officers acknowledge having read same and represent to the City that Permit Holder, as a Not For Profit Corporation, has the authority to execute this Permit and fully agrees to all terms and conditions herein.
