EMERGENCY MEDICAL SERVICES ALS FIRST RESPONDER AGREEMENT EXTENSIONEMERGENCY MEDICAL SERVICES ALS FIRST RESPONDER AGREEMENT EXTENSION OCTOBER 1, 2010 PINELLAS COUNTY EMERGENCY MEDICAL SERVICES AUTHORITY 12490 Ulmerton Road Largo, Florida 33774 ,l Emergency Medical Services ALS First Responder Agreement Extension EXTENSION OF ALS FIRST RESPONDER SERVICES AGREEMENT AGREEMENT made this _ --? 1 - day of September 2010, between CITY OF CLEARWATER, a Florida municipal corporation ("Contractor") and the PINELLAS COUNTY EMERGENCY MEDICAL SERVICES AUTHORITY, a special district ("Authority"). In consideration of the mutual benefits set forth below, the parties agree as follows: 1. Contractor currently contracts with the Authority to provide Advanced Life Support (ALS) First Responder Services. 2. The ALS First Responder Services Agreement between the Contractor and the Authority, dated September 22, 2009 (the "Agreement") is set to terminate on September 30, 2010. 3. The term of the Agreement shall be extended to and including September 30, 2011. 4. Contractor and Authority agree that upon approval of this extension, except with regard to the extension of the term of the Agreement and the matters set forth in paragraph 5, all terms of the Agreement will remain in full force and effect as if the original term of the Agreement extended to September 30, 2011. 5. The Authority funded and authorized ALS First Responder Units, listed as Appendix "A", and the Annual Compensation for FY10-11, listed as Appendix "B", are attached hereto. r Emergency Medical Services ALS First Responder Agreement Extension IN WITNESS WHEREOF, the parties hereto, by and through their undersigned authorized officers have caused this Extension of First Responder Services Agreement to be executed on this day of 06kk; e. , 2010.
