The Development Review Committee at its meeting of July 11, 1991. approved the above described Reva i ttiion
of the Certified Site Plan subiect to the previous conditions of the Site Plan certified on Nov. 2. 1990 with the
following amended condition #I1 to read as follows:
11. The requisite initial building permit shall be procured by May 2, 1992 and all requisite certificates of
occupancy shall be obtained within three years from the date of original certification of November 2, 1990.

The Development Review Committee at its meeting of July 12, 1990 approved the above described site plan subject
to the following conditions:
1. Any vacation of the alley due to the location of the compactor or dumpster must be approved prior to
certification of the site plan and shown as such on the site plan (On October 4, 1990, the City Commission
approved the vacation of the north 30 feet of the 16 foot wide alley).
2. Prior to issuance of a building permit a copy of the approved SWFWMD permit must be submitted to Public
Works Department.
3. The required landscape buffers shall be dimensioned on the site plan prior to certification.
4. A traffic impact study in accordance with the City's concucrency requirements shall be submitted to the City
Traffic Engineer for approval prior to certification of the site plan.
5. The proposed water distribution system, subject to approval of the Utilities Engineer, must be shown on the
final site plan prior to certification (As per the Utilities Engineer, this item will be handled at time of
application for building permit).
6. Reduced pressure backflow prevention devices must be installed on the water services and double detector
check valves must be installed on the fire lines by the City and shown as such on the site plan prior to
certification (As per the Utilities Engineer, this item will be handled at time of application for building
permit).
7. Prior to certification, the Dumpster Requirement Form must be completed and returned to the City Sanitation
Department.
8.. Prior-to a C.O., the dumpsters and compactor must be visually screened. Provide gates with a minimum
12' clear opening to the front for access by Sanitation trucks.
9. Fifteen foot comer easements for utilities and sidewalks at Jones/Osceola and Osceola/Drew shall be
provided, to be dedicated prior to the issuance'of a building permit.
10. Signs and fencing and walls are subject to separate review and permitting processes.
11. The requisite initial building permit shall be procured within one year from date of certification and all
requisite certificates of occupancy shall be obtained within three years from the date of certification.
As conditions #1,3,4,7 have been met, please certify the attached plans/plats subject to conditions #2,5,6,8,9,10,11
listed above and distribute.
