Dear Fowler & Assoc :
The Planning Staff has entered your application into the Department's filing system and assigned the
case number: FLD2005-07069. After a preliminary review of the submitted documents, staff has
determined that the application is Incomplete with the following comments.
1. Provide a completed "Affidavit to Authorize Agent" form, which is page 7 of the application.
2. Revise scale of drawings to indicate 1" = 20 feet.
3. Provide on Page 1 of the application both parcel numbers for the subject property.
4. Provide a STORMWATER PLAN, including the following:
Existing topography extending 50 feet beyond all property lines;
Proposed grading including finished floor elevations of all structures;
All adjacent streets and municipal storm systems;
Proposed stormwater detention/retention area including top of bank, toe of slope and
outlet control structure;
_ A narrative describing the proposed stormwater control plan including all calculations
and data necessary to demonstrate compliance with the City manual.
Signature and seal of Florida Registered Professional Engineer on all plans and
calculations.
5. Provide a COPY OF PERMIT INQUIRY LETTER OR SOUTHWEST FLORIDA WATER
MANAGEMENT DISTRICT (SWFWMD) PERMIT SUBMITTAL (SWFWMD approval is
required prior to issuance of City Building Permit), if applicable
6. On the application form on Page 4, acknowledgement of stormwater plan requirements
(Applicant must initial one of the following):
Stormwater plan as noted above is included
Stormwater plan is not required and explanation narrative is attached. At a
minimum, a grading plan and finished floor elevations shall be provided.
7. Provide one signed and sealed survey of the existing 1310 Cleveland Street (southern parcel).
An unsealed version has been submitted.
8. Provide a TREE SURVEY (including existing trees on site and within 25' of the adjacent site,
by species, size [DBH 4" or greater], and location,
including drip lines and indicating trees to be removed) - please design around the existing
trees.
9. Revise the required parking ratio from four spaces per 1,000 square feet to five spaces per
1,000 square feet and the required number of spaces from 11 to 13 spaces.

10. Provide a GRADING PLAN.
11. Overall property will be considered a corner lot. As such, front setbacks of 25 feet are
required on both street frontages and side setbacks of 10 feet will be required along the other
property lines. No reduction to the front setback has been requested on the Fredrica Drive side
nor along Cleveland Street.
12. Show required sight triangles at all driveways, as well as the intersection, measured along the
property lines and edges of driveways.
13. Show the location of existing public and private utilities, including fire hydrants, storm and
sanitary sewer lines, manholes and lift stations, gas and water lines.
14. Dimension the width of all existing and proposed driveways.
15. Depict by shading or crosshatching all required parking lot interior landscaped areas.
16. Provide the location of all outdoor lighting fixtures.
17. Provide in the site data table the existing, proposed and required impermeable surface ratio
(I.S.R.).
18. Delineate and dimension all required perimeter landscape buffers (15 feet along Cleveland
Street; 10 feet along Fredrica Drive; 10 feet along the north; five feet along the east).
19. Show all required curbing of landscaped areas and wheel stops.
20. Show all existing trees on-site and immediately adjacent to the site, by species, size and
locations, including driplines (as indicated on required tree survey).
21. Indicate on the landscape plan the quantities of all existing and proposed landscape materials
within each planting area.
22. Code requires organic mulch. Crushed shell is not an approved mulch. Revise.
23. Provide in the site data table the square footage and percentage of the vehicular use area for
interior landscaping.
24. Provide irrigation notes.
25. Dimension the width of the foundation landscape area on the south side of the building.
26. Any reduction to the amount of interior landscape area, width of the foundation landscape area
or landscape buffers will require the submission of a Comprehensive Landscape Program
application, specifying the reduction or modification desired and providing the justification for
such.
27. Provide the north and east building elevations on the 24" x 36" sheet.
28. Provide the reduced east building elevation.
29. Provide the proposed colors of the building on building elevations, including the gas canopy,
and provide color samples/chips (one copy of the color samples/chips).
30. It appears that the proposal includes relocating the existing freestanding sign to the
intersection. If so, indicate the actual depiction of the sign (not a circle) and provide the
proposed setback from property lines. Please note, the proposed location will not be
supported.
