OlL~~~ SCOPE OF CHANGE: THIS CHANGE ORDER ACCEPTBREVISIONS TO THlE-CONTRACT This change order provides for the final Amendment # 3 to the Engineering Services Agreement dated Jan. 14, 1988 with Tampa Bay Engineerir.g, Inc.. increasing the approved agreement amount $9,925. for a new total agreement amount of $434,148.00. This amendment provides for the additional engineering services by Tampa Bay, at the hourly rates established in the agreement, nece~sary to close the Countryside Blvd. widening project. The specific services are to provide final Construction Engineering and Inspection on the ongoing construction contract with the R. E. Purcell Construction Company and to submit all certification requirements to the various regulatory agencies which have issued permits on this project. Tampa Bay Engineering agrees that no additional requests for payment will be made to the City of Clearwater on this project beyond the proposed amendment #3 for the completion of project closure. The city of Clearwater will provide the following engineering services assistance in the closure of this project: field testing of asphaltic concrete density during the final application of paving by Purcell, obtaining as-built field dimensions as necessary on drainage and paving structures and the transfer of this field information onto Auto CADD drawings provided by Tampa Bay Engineering. The new agreement total will be $434,148.00. BACKGROUND FOR CHANGE ORDER: In January 1988 the City Commission approved an agreement with Tampa Bay Engineering, Inc. to provide engineering design services for the widening of Countryside Blvd. In November, 1990, the City commission approved amendment #2 to the agreement to provide for Construction Engineering and Inspection services for the construction of the project. The project is almost complete with Purcell performing the final paving applications on the east side of Countryside Blvd. at this time. The funding provided for in amendment #2 is nearly expended because of additional inspection and design services requested by the City such as redesigning the Countryside Mall and Inverness Condominium Complex intersection; coordination with Florida Department of Transportation (FDOT) for the u.s. 19 connection; addition of the 6" water main installation; paving of existing 6 foot wide medians with concrete which included obtaining a letter of exemption from the Southwest Florida Water Management District; design of additional guardrail at Winding Woods Drive. It is now apparent that funding is insufficient to provide for the necessary engineering services to close the construction contract and submit as-built certifications to the various regulatory agencies. The purpose of this final change order is to prpvide for these engineering services in a cooperative effort by Tampa Bay Engineering and the Engineering Division of Public Works. As shown in the attached change order form, the City will provide the services of field density testing for the Purcell paving operations, obtain as-built field dimensions and perform drafting services to transfer this field information to continued: Page 2 continued: . Tampa Bay will provide all necessary Construction Engineering and Inspection services to final the City's construction contract with Purcell and will check and submit all as-built certifications to the various regulatory agencies which have issued permits on this proj ect. These services will be provided by Tampa Bay with no further invoice requests beyond the above final agreement amount. ADMINISTRATIVE SERVICES DEPARTMENT REVIEW I recommend approval based on the administrati ve change order criteria: 1. Increases do not exceed $10,000 over Commission approved amount on a cumulative basis. 2. Change does not change the scope of the project. 3. Price increases do not require additional appropriation to the project. 4. Contract price decreases may be approved without limitation. 5.
