hereinafter referred to as the Department, and City of Clearwater

WITNESSETH:
WHEREAS, the Department and the Ageney heretofore on the 18th
entered into a Joint Participation Agreement; and
WHEREAS, the Ageney desires to aecomplish eertain projeet items as outlined in the Attachment "A" appended
day of April
2005
hereto; and
WHEREAS, the Department desires to partieipate in all eligible items for this project as outlined in Attaehment
"A" for a total Department Share of $202,800.00
NOW, THEREFORE THIS INDENTURE WITNESSETH: that for and in consideration of the mutual benefits to flow
from eaeh to the other, the parties hereto agree that the above deseribed Joint Participation Agreement is to be amended
and supplemented as follows:

DESCRIPTION OF SUPPLEMENT (Include justification for cost ehange):
This is a Pre-qualified Supplemental Agreement adding 52,800 dollars in Department funding and 13,200 dollars in local
funding in fiseal year 06/07. The revised total project eost will be 253,500 dollars. The added cost is for upgrading the
building roof and site preparation work.
