BE IT ORDAINED BY THE CITY COMMISSION OF THE CITY OF CLEARWATER, FLORIDA: Section 1. Sections 76.21, 7643 and 76.25, Code of Ordinances, are amended to read: Sec. 76.21. Application. (a) Any person required desirl g to secure a permit under this article shall apply therefor in writing over his or her signature to the chief of police on forms provided by the city, and such application shall state: (1) The name and address of the applicant; (2) The name and address of the person by whom employed; (3) The length of service of such applicant with such employer; (4) 'The place of residence and nature of employment of the applicant during the last preceding year; (5) The nature or character of the goods, wares, merchandise or services to be offered by the applicant; and (6) The physical description of the applicant, to include race, sex, height, weight, eye color, hair color, complexion and body build. (b) The applicant shall display a driver's license or other sufficient evidence of the identity of the applicant at the time the application is filed.
