Mr, Feldhaus for f�tness ta retum to duty. Dr. O'Nesf conclucled Mr. Feldhaus is unable tn

An August 3, 7999, Edwin C. Feldhaus grak�bed a large, falling, propane tank, tried

consultation a planned ta refer Mr. Feldhaus ta Dr. lyer, Chronic Pain Management, for
further evaluation and treatment of patient`s law pack pain. Subsequent office visits
occurred on July 2�, Octaber 1 1, November 8, 2000, January Z 1, A�rii T 1, anci July 20,

" On Apri� 25, 2000, Jayam K. lyer, M.D., of Creative Nealth C�nter, hac! an initial
consu�tation regarding low back Pain and a sore rigi�t shoulde�. 5ubsequent o�fice visit�

vehicle as he copes with stressars by drinking alcohal,

Mayer, M.D. admitted him to Morton Pian Hospital for crisis stabilizatian, neuraphyschiatic

Mr: �eldhaus applied for a Disability Pensivn o� Aug��st 7, 2001, but did nat

� after review of the February �, 2002 Functional Capacities Assessm�nt for Mr. FeEdhaus,

�� that Mr. �efdhaus is tctally disa�led fram performing the ciuties af a Gas Technician but is
capable of perfarming sedentary work.
In his .January 16, 2002 letter, Psychiatrist Jake Aslanian, M:�. stated he had
perf�rrr�ed an !ME on �dwin Feldhaus on February 7, 2002, and indicateci the claimant is
not totally disabled from performing the duties af a Gas Technician II as a resuEt of his
psycholagical conditian.

Genera! I�iospital �Rehabilitation Se�vicos statQd a�unctional Capacities Assessmer�t was
performed for Edwin FeEdhaus on .lanuary 31, ZOn2. Hesults indica#ed he is functioning
with same deficits at the UGHT duty leuel. The W,O.R,K. Center�interdiscipEinary

said records indicate Mr. Feldhaus' injury is secondary ta his disfc disease.

said Mr. Feldhaus had injured his bac�C when trying ta assist anather when a LP tanlc began

� to.this accident. Mr. Feldhaus was limited to manual labor and canfined to �f�e yard. He

I�� was felt Mr. Feldhaus is disabled and that no meaningful wark is a�ailable far him
with the City. It was.stated documents support that Mr. Feldhaus had suffered menta!
hsalth �aroblems and substance abuse issues prior ta his ir�jury. ft was suggested Mr.
Feldhaus could perform maaningful wark if his back injury was not compounded by mflntal
health �roblems, In response to a question, Mr. �'Raurke� said ths City never had
mandated �AP (Employee Assistance Program} psychological therapy prior to ��r. Feldh�us'
injury. 5ince the injury, the City has refsrred him far substance abuse, It waa felt the City
should have addressed mentai issues prior to Mr, Feldhaus' accident. It was suggested
Mr. Feldhaus` psychologicak problems may have contribut�d to the accident. It was stated
Mr. Feldhaus could perform few Job duties based solely an his physical condition which
limits his ability ta stand ar sit ta 10 minutes at a�imQ. Mr. 0'Raurke said the City is able
to make reasonable accommacfatians far Mr. Feldhaus in t�e short term, but not in the long
term.

campared the relative n�t value of current and proposed (�.g., 1.5% COLA} hazardaus
duty and non-hazardo�s pension benefits to those provided by selected government
agencies in NorthlCentral Florida and tF�e Tampa Bay area. The survey results indicated
that City hazardous duty employees were ranked in the � st quartile or ttiird - Fire and
fourth - Police res}�ectively, aut o# the 19 surveyed agencies. By camparisan, the City's
nan-hazardous er�ployees were ranfced in the third quartile ar sixth out af the 10 surv�yed

and information on in�estment managers. The #irm has extensive knowledge and
experience with all c�stodians. Calian's search #ee is $25,000, plus travel expenses, not
ta sxceed $5,000. Unti[ now, Caflan's fees are paid by directing investment trades
through a selected br4ker. Plam m�ney managers use this selected broker anly if they
canr�ot get better execution someplace else. If necessary, the Pensian Plan may have to
supplement these directed trades with a cash payment.
The cost of cammittee member travel to in�astigate/researoh t�p-rankecf custodians �
is es#irnated at $3,500. The locatian of custodians, and the number requiring research will
affec# the actual cost. Staff also may use the trip to visit maney managers located in a
city staff already is �isiting. The In�estment C�mmittee wil� come back to the Tr�stees in
