The motion was duly seconded by Fred Ratley and carried unanimously. With respect to the'request of Emanuel Hutchinson, Solid Waste Worker, Solid Waste Department, for a job-connected disability pension, thc Committee received a new letter from Dr. Craig Schwartz. Mr. Hutchinson has been deemed totally and permanently dili'abled by Workers' Compensation. His Workers' Compensation file' was in order. Based on the new information and previous documentation submitted, Ed Hooper moved that Emanuel Hutchinson be granted a job-connected disability pension effective on a date to be determined based on Mr. Hutchinson's disability that resulted from on~the~job circumstances which is described as a diskectomy and cervical interventions" said disability making him permanently unable to perform his job, and documented by medical statcments 0 f Dr. Robert Harris, datcd December 20, 1994; Dr. Donald V. Graham, dated June 221 1994; and Dr. Craig Schwartz, datcd August 161 1995. The motion was duly seconded by Richard Breest and carried unanimously. The Committee again considered the request from Thomas Coleman, Fire District Chi c f, Fire Department, for a job-connected disability pension based on a blood' condition that has resulted from his employment as a ,Firefighter. An IME report was receivcd from Dr. Peter' Kennedy. Mr. Coleman's Workers' Compensation file was in order. For the record, Mr. Coleman advised the Committee that Fire Chief Bob Davidson assured him that there is no other position in the Fire Department for Mr, Coleman other ,than that outlined in his job description. It was also stated for the record that Mr, Coleman has always been a non-smoker and has' responded to approximately 75 fires per year during the course of his employment. Ed Hooper moved that Thomas Coleman be, granted a job-connected disability pension e ffec ti v e on a dute to be determined based on Mr. Coleman's disability that resulted from on-the-job circumstances which is described as polycythemia, said disability making him' permanently unable to perform' his job1 and documented by medical statements of Dr. Luis Fernandez, datcd If::f.~, July 26, 1995; Dr. Jeffrey Paonessa, dated June 201 1995; and Dr. Peter Kennedy, dated August 29, ~ 1995. Puge 2 The Committee again considered the request from Joseph Stonelake, Marine Il'ncillty Ope,rator, Marine Department, for a job-connected disability pension. Attempts to find ,allernntive employment for Mr. Stone lake have been unsuccessful. His Workers' Compensation file is in order. Ed Hooper movcd that Joseph Stonelake be granted a job-connected disability pension effective on a datc to be dctcrmined based on Mr. Stonelake's disability that resulled from on-the-job circumstances which is describcd as a disk herniation at L5-S 1, said disability making him permanent,ly unable to perform his job, and documented by medical statements 0 f Dr. Joseph Sena, dated July 31, 1995; Dr. Nelson Smith, dated June 30, 1995; Dr. R. W. Springstead, dated Junc 20, 1995; Dr. William Cubides. datcd June 6. 1995; and Dr. Robcrt G. Hamilton. dated ,July 17. 19951 and based on unsuccessful attempts to find alternative employment. The motion ,was duly seconded by Richard Breest and carried unanimously. The, Committee again 'considered the request from Timothy DeBord, Firefighter, Fire De par t rn en t, for a job-connected disability pension based on a back injury suffered in the line of duty. Mr. Ratley raised a question as to whether or not Mr. DeBord should be sent for an IME. Mr. DeBord has been given an MMI date of October 1. 1995. based on the fact he is continuing to receive therapy. Fred Ratley moved that to benefit all concerned Mr. DeBord be sent. for an IME. The motion was duly seconded by Richard Breest and carried unanimously, The Committee considered the request from Gary Milarck, Marine Facility Operator, Marine Department, fora job-connected disability pension based on a back injury received in the line of duty. Mr. Milarck submitted letters from two doctors which indicate that he has a lifting restriction of no more than 50 pounds. Mr. Milarck is currently working at the toll plaza but was moved to the fuel dock at the marina to see if he could handle the duties there. Bill Held,' Harbormaster, provided a job description for the duties at the fuel dock which does not indicate anything that requires the ability to lift more IIHln 50 pounds, Mr. Hooper stated he had a problem with Mr. Milarck1s documentation in that there is nothing to support the claim that he cannot do the job at the fuel dock. Mr. Stonelake, an employee i n the same job c1ass, advised the Committee that there are duties at the fuel dock that do rcq u i re the ability to lift more than 50 pounds. The Committee will take no action on this request until more substantiating documentation is received. Mr. Stonelake will advise Mr. Milarck of the Committee's decision. Mr. Milarck's Workers' Compensation file was in order. Thc Committee considered the request from William Matlin, Mechanic II, Fleet Maintenance Division, General Services Dcpartmcnt,for a job-connected disability pension based on a back injury. After review of his letters and a lengthy discussion, Mr. Hooper advised Mr. Matlin that his letters did not support his claim for a job-connected disability pension partly due to the fact that there is no documentation of any injury to his back prior to January 3, 1995, and his letters state that he has degenerative disk disease compounded by a back strain/sprain as, a result of the January incident. The Committee members agreed that Mr. Matlin has a back problem with permanent restrictions; however, there is not enough evidence to support his claim that it is job-connected. Maintenance Worker, Solid Waste Departmcnt,for a job-connected disability pension based on a knee injury. Ms. Traub originally injured her knee in 1992 but' reinjured it in 1993.
