TRUSTEES OF THE EMPLOYEES' PENSION FUND MEETING
June 2, 1997
The City Commission of the City of Clearwater, meeting as the Board of Trustees of
the Employees' Pension Fund, met in regular session at City Hall, Monday, June 2, 1997 at
1 :00 P.M., with the following members present: .
ITEM #2 - Minutes:
Trustee Johnson moved to approve the minutes of the April 28, 1997, meeting.
The motion was duly seconded and carried unanimously.
ITEM #3 - ReCluest for Acceptance into Membership:

The City Manager presented the recommendation of the Pension Advisory
Committee to approve membership for the employee(s) listed below:
a) Madona Hoke
b) Stacey Sterling
c) Sara E. Agrait
d} Yolanda Franklin
e) John Clarkson
f) Rudy V. Chassion
g) Traci L. Tennell
Trustee Hooper mcved to accept the recommendation of the Pension Advisory
Committee. The motion was duly seconded and carried unanimously.
ITEM #4 - ReCluest for Pension:
The City Manager presented the recommendation of the Pension Advisory
Committee that Robert Davidson be granted a regular pension under Section(s) 2.393 and
2.397 of the Employees' Pension Plan.
Robert Davidson w'as employed on July 26, 1 964, and his pension service credit is
effective on that date. His pension will be effective on August 1, 1997.

Mr. Davidson's pension was approved by the PAC on Maya, 1997. Section 2.393
(p)provides for normal retirement eligibility when a participant has completed twenty years
of credited service in a type of employment described as "hazardous duty" and further
specifically defines service as a Fire Chief as meeting the hazardous duty criteria.
Based on an average salary of approximately $74,600 per year over the past five
years, the formula for computing regular pensions, and Mr. Davidson's selection of the
100% Joint & Survivor Annuity, this pension will approximate $ 65, BBB annually.
Trustee Hooper moved to accept the recommendation of the Pension Advisory
Committee. The motion was duly seconded and carried unanimously.
ITEM #5 - Request for Pension:
The City Manager presented the recommendation of the Pension Advisory
Committee that Robert Heine, Sr. be granted a regular pension under Section(s) 2.393 and
2.397 of the Employees' Pension Plan.
Robert Heine, Sr. was employed on June 1 3, 1977, and his pension service credit is
effective on that date. His pension will be effective on July 1, 1997.

Mr. Heine's pension was approved by the PAC on May B, 1997. Section 2.393 (p)
provides for normal retirement eligibility when a participant has completed twenty years of
credited service in a type of employment described as "hazardous duty" and further
specifically defines service as a Police Officer as meeting the hazardous duty criteria.
Based on an average salary of approximately $39,272 per year over the past five
years, the formula for computing regular pensions, and Mr. Heine's selection of the 60%
Joint & Survivor Annuity, this pension will approximate $22,255 annually.
Trustee Clark moved to accept the recommendation of the Pension Advisory
Committee. The motion was duly seconded and carried unanimously.
ITEM #6 - Request for Pension:
The City Manager presented the recommendation of the Pension Advisory
Committee that Gregory Hoover be granted a regular pension under Section(s) 2.393 and
2.397 of the Employees' Pension Plan.
Gregory Hoover was employed on October 10, 1977, and his pension service credit
is effective on that date. His pension will be effective on November 1, 1997.
Mr. Hoover's pension was approved by the PAC on May 8, 1997. Section 2.393 (p)
provides for normal retirement eligibility when a participant has completed twenty years of
credited service in a type of employment described as "hazardous duty" and further
specifically defines service as a Fire Lieutenant as meeting the hazardous duty criteria.

ITEM #8 - ReQuest for Pension:

Based on an average salary of approximately $43,767 per year over the past five
years, the formula for computing regular pensions, and Mr. Hoover's selection of the 100%
Joint & Survivor Annuity, this pension will approximate $23,842 annually.
Trustee Johnson moved to accept the recommendation of the Pension Advisory
Committee. The motion was duly seconded and carried unanimously.
ITEM #7 - ReQuest for Pension:
The City Manager presented the recommendation of the Pension Advisory
Committee that William James be granted a regular pension under Section(s) 2.393 and
2.397 of the Employees' Pension Plan.
William James was employed on December 13, 1 971, and his pension service credit
is effective on that date. His retirement was effective on May 1, 1997.
Mr. James' pension was approved by the PAC on May 8, 1997. Section 2.393 (p)
provides for normal retirement eligibility when a participant has completed twenty years of
credited service in a type of employment described as "hazardous duty" and further
specifically defines service as a Firefighter as meeting the hazardous duty criteria.

Based on an average salary of approximately $37,912 per year over the past five
years, the formula for computing regular pensions, and Mr. James' selection of the Joint &
Survivor Annuity, this pension will approximate $26,450 annually.
Trustee Hooper moved to accept the recommendation of the Pension Advisory
Committee. The motion was duly seconded and carried unanimously.
The City Manager presented the recommendation of the Pension Advisory
Committee that Patricia Voorhees be granted a regular pension under Section(s) 2.393 and
2.397 of the Employees' Pension Plan.
Patricia Voorhees was employed on May 13, 1968, and her pension service credit is
effective on that date. Her pension will be effective on June 1, 1998.
Ms. Voorhees' pension was approved by the PAC on May 8, 1997. Section 2.393
(p) provides for normal retirement eligibility when a participant reaches age 55 and has
completed twenty years of credited service or has completed thirty years of credited
service. Ms. Voorhees qualifies under both criteria.
Based on an average salary of approximately $26,211 over the past five years, the
formula for computing regular pensions, and Ms. Voorhees' selection of the 50% Joint &
Survivor Annuity, this pension will approximate $22!506 annually.

The City Manager presented the recommendation of the Pension Advisory
Committee that Billy Johnson be granted a regular pension under Section{s) 2.393 and
2.397 of the Employees' Pension Plan.
Billy Johnson was employed on December 26, 1973, and his pension service credit
is effective on that date. His pension was effective on June 1, 1997.
Mr. Johnson's pension was approved by the PAC on May 8, 1997. Section 2.393
(p) provides for normal retirement eligibility when a participant reaches age 55 and has
completed twenty years of credited service or has completed thirty years of credited
service. Mr. Johnson qualifies under the twenty years of service and age 55 criteria.
Based on an average salary of approximately $24,046 over the past five years, the
formula for computing regular pensions, and Mr. Johnson's selection of the Joint &
Survivor Annuity, this pension will approximate $15,384 annually.
Trustee Johnson'moved to accept the recommendation of the Pension Advisory
Committee. The motion was dUly seconded and carried unanimously.
