A meeting of the Clearwater Police Supplementary Pension Fund was held November 4, 1976 in the Conference Room of the Police Department, 644' Pierce Street, Clearwater, Florida. '1'\ Also present'were Marybeth Lavallee, Assista~t City Attorney, and Betty Ward, Chief Daniels' Secretary_ ,\':. Officer Kaminskas made a motion that Donald Hibbard serve as Acting Chairman for this meeting. Officer Sedrick 'seconded the motion. . () Motion carried unanimously. Acting Chairman Hibbard called the meeting to order at 10:55 A.M. and called for the approval of the Minutes of the meeting held September 23, 1976. Officer Kaminskas made a motion that the Minutes of the meeting held September 23, 1976 be approved. Motion seconded by Officer Sedrick. Motion carried unanimously. " '"."~, ~ , , ~ ' ' If,' ,~ ~ Chairman Hibbard called upon Chief Daniels to explain Item Number One on the agenda, which was a request from Police Officer Edwin Kent (Retired) to change his deferred annuity with Provident Insurance Company to that of a monthly supplementary pension. Chief Daniels advised that Officer Kent at the time of his retirement had chosen to 'elect the Deferred Annuity at age 65, but since that time has requested that this be changed to a monthly check. There would be no additional cost to the Fund to make this requested change. Mr. Kent wou,ld receive $74.20 per month under Contract NO. 887150. Mr. Kent has signed the contract and has elected the cash Refund Annuity figure of $74.20 per month. Officer Sedrick made a motion that Officer Kent's request be approved.
