"~.,..I ' MINUTES OF MEETING OF BOARD OF TRUSTEES OF CLEARWATER POLICE SUPPLEMENTARY PENS ION FUND '~ A meeting of the Clearwater Police Supplementary Pension Fund was, held September 23, 1976 in the Conference Room of the Police Department, 644 Pierce Street, Clearwater, Florida.' ,) , 'J l' The following Board members were present: GABRIEL CAZARES, .Chairman FRANK DANIELS, Secretary DONALD 'HIBBARD RAY KAMINSKAS WILLIAM SEDRICK .' " A1so'present were Marybeth Lavallee, Assistant City Attorney,and Betty ~ard, Chief Daniels' Secretary. i, ;:) Mayor Cazares, Chairman, called the meeting to order at 2:00 P.M. He requested the approval of the minutes of the meeting held March 9, 1976. Donald Hibbard made a motion that the minutes be approved. William Sedrick seconded the motion. Motion carried unanimously. The Board welcomed Marybeth Lavallee, Assistant City Attorney, to the meeting. , . Chief Daniels presented an invoice from Provid~ Insurance Company in the amount of $83,146.47 to the Boa~ for payment. He explained that the invoice covers the cost of the life insurance premiums for present participants in the Supplementary Pension at the anniversary date of the Plan, September 17, 1976. Ray Kaminskas made a motion that the invoice be approved for payment. Motion seconded by Donald Hibbard. Motion carried unanimously. Chief Daniels submitted an invoice from the Bank of Clearwater in the amount of $8.00 for Safe Deposit Box rental due September, 1976. Donald Hibbard made a motion'that the in~oice be approved for payment. Motion seconded by William Sedrick. Motion carried unanimously.
