MINUTES
MEETING - Clearwater Police Supplementary PcnsionF'und,
held at 10:00 A. M., October 5, 1971' in Office of
Police Chief Frank Daniels
Those members present were Chai.rman Mayor H. Everett Hougen, Donald
Hibbard, Jerry Roberts, Blaine LeCouris, F'rank Daniels and Guy L. Kennedy.
The meeting was called to order at 10:00 A. M. by Chairman Hougen. Major
LeCouris made a motion that the minutes of the meeting held on May 14, 1971
be approved as written. Motton seconded by Donald Hibbard. Motion
carried unanimous~y.
The Chairman advised that the first item on the agenda was the approval of an
invoice from Provident Insurance' Company, in the amount of $38, 873. 36"
covering the annual premium for participants. Chief Daniels stated that a
tape had been run on the invoice and the figures were accurate. Mr. Hibbard
made a motion that the invoice be approved and payment submitted accordingly.
MC?tion seconded by Jerry Roberts. Motion carried unanimously.
The second item on the agenda was a request from Patrolman Loman Mack for
pension benefits. Mr. Kennedy reported that a letter had been sent to Provident
requesting the pension benefits for Loman Mack. Provident, in their letter of
August 27, 1971, had set forth their computations, which Mr. Kennedy forwarded
to Har;ry Boggs, of the State Treasurer's Office in Tallahassee, for his review
and recommendation. Mr. Boggs advised Mr. Kennedy by phone that he questioned
Provident's computations and that the Board should have them rechecked by
Provident. This was done and on September 16th Provident sent their new com-
putations, which have been forwarded to Mr. Boggs for approval. Mr. Hibbard
made the motion that the Board of Trustees approve Loman Mack's pension and
authorize the expenditure of the necessary funds to purchase the required' annuity
in accordance with the benefits as computed by Provident, subject to formal
written approval of said benefits by Mr. Harry Boggs of the State Insurance Company's
Office in Tallahassee. Motion seconded by Major LeCouris and passed unanimously.
Third item on the agenda was a request from Sergeant Floyd Murrell for pension
benefits. Chief Daniels read a letter dated July 30, 1971 from Dr. V. L. Hagan,
a letter dated May 14, 1971 from Dr. H. G. Siele, Jr., and a letter dated .July 26,
1971 from Dr. Carl H. CaIman. ,Guy Kennedy read the State Statute with respect
. to disability under the supplementary pension. Mr. Kennedy recommended that the
Board request an up- to-date letter from one of the physicians, setting forth Sgt.
Murrell's disability. Mr. Hibbard made the motion that Sgt. Murrell be required
to make an appointment with Dr. Sick for re-examination and an up-to-date letter
be furnished the Board of Trustees as to the disability. Motion seconded by Jerry
Roberts. Motion carried unanimously.
There being no further business. the meeting adjourned at 10:45 A. M.
