',' ._". ..~I....,~...n..r-:'I'.~: " MINUTES I. o MEETING - Clearwater Police Supplamentary Pension Fund. Held May 14J 1971 in Office of Police Chief Frank Daniels. Those members present were: Donald HibbardJ Jerry HobertsJ ,Blaine LeCourisJ Frank Daniels, and Guy L. Kennedy. Mayor H. Everett Hougen, Chairman, was unable to be present. Mr. Hibbard made a motion that Frank Daniels be appointed Acting Chairman in the absence of Mayor Hougen. Motion seconded by Jerry Roberts. Motion carried . . unani.ri1ous ly. The meeting was called to order at 10:30 A. M. by Acting Chairman Daniels. Chief Daniels asked Mr. Kennedy, Assistant City Attorney. for a report on Captain Elwood Lomax'S request for disability pension benefits. .Mr. Kennedy reported that the Board had received a letter from Provident Insurance Company dated April 20J 1971 setting forth certain annuities for Captain Lomax and this information was thereafter furnished to Mr. Harry Boggs of the Insurance Commissioner's Office in Tallahassee for review by his office. Mr. Kennedy also stated that Mr. Boggs had responded to his letter stating that his office found Provi,dent's ~tatement to be correct as outlined by Provident. .-") 1;,..... Mr. Kennedy stated that Captain Lomax has requested retirement under Florida Statutes 185 and specifically sub-section l8-disability. Chief Daniels stated that he had received letters from Dr. William R. Tench and Dr. J. O. Norton stating that they have each examined Captain ~omax and found him to be totally and permanently disabled. " Major LeCouris made a motion that the Boar'd accept the statements of the two duly recognized physicians, Doctor Tench and Doctor NortonJ and that the requested pension for Captain Lomax be accepted by this Board. under the disability provision. and that the Secretary be authorized to transfer the necessary funds and take all necessary action needed to implement this. Mr. Hibbard seconded the motion. Motion carried unanimously. Chief Daniels informed the Board that the amount needed . to purchase the annuity would be $17. 904. 54. There being no further business, Mr. Hibbard moved that the meeting be adjourned, seconded by Jerry Roberts. Meeting adjourned at 10:55 A. 1\1. .
