" (] >."..' MINUTES y of Meeting held 10:00 A. M. J.'anuary 22. 1971 in 'Office of Police Chief Daniels. Clearwater Police Supplementary Pension Fund . Meeting was brought to order by Chairnui.n H. Everett Hougen. Those'present were: Frank Daniels. Secretary. Officer Jerry Roberts. Mr. Donald Hibbard. Major Blaine LeCouris and Guy L. Kennedy, JI,'.. Assistant City Attorney. , . . , , Mr. Hibbard made a motion that the reading of the minutes of the last meeting be waived and that the,minutes be accepted as submitted. Motion seconded by Major LeCouris. Motion carried, unanimously. " \ ,..j Mayor Hougen opened the meeting with the one agenda item, "Request from Wallace B. Mackey for pension benefits under the Police , Supplementary Pension Plan". and Chief Daniels advis ed Mayor Hougen that Mr. Kennedy was prepa'red to report on this ml1tter. Mr. Kennedy reported that he had communi.cated with the Provide!1t Life and Accident Insurance Company regarding Major Mackey's request and that Provident has submitted the computations for the three options, one of which Major Mackey could choose. Mr. Kennedy also advised that correspondence had been received ,from' Mr. Harry Boggs of the Insurance Commissioner's Office in Tallahassee stating that the computations made by Provident' appear to be in order according to their records. Mr. Kennedy felt that it would be in order for the Board to consider offering to Major Mackey his choice of these three options. and that the Board furnish him the necessary forms so that the matter can be processed. Major LeCouris made a motion that the options as outlined by Provident be sent to Major Mackey, and that upon receipt of Major Mackey's option choice, the Secretary be authorized to transfer and expend the necessary funds to accomplish this. and to execute the necessary paperwork.
