LIBRARY CARDS AND PATRON DATA ENTRY

Patron must present 1.0. with his/her current address.
1. If the patron is paying non-resident fee, 1.0.
may be presented next time in if they have none
currently.
2. Acceptable 1.0. for patrons 18 and up:
a. Drivers license
b. state 1.0.
c. Voter's I.O.
1. Under section marked HeITY", if it is.
marked "OW" then it is an address within
the incorporated boundaries of the city
of Clearwater.
2. If the section marked "CITY" has an "X"
or other letters, then it is a non-
resident address and the patron must pay
non-resident fee for a card.
d. Mail with recent postmark
e. Checks
f. Lease, rental receipt, or rental agreement
with address indicated
g. Tax receipt
h. Gaylord library card with current address
i. Letter from landlord indicating address
j. Auto registration
k. city employee 1.0.
1. Any official document indicating address
but not applications where patron has
entered their own address.
m. Motel key (must bring better 1.0. next
time in)
n. Verbal or written assurance from a city
resident that the person is a guest (city
resident must have a library card or
acceptable 1.0.)
o. Last resort: call property appraiser,
patron must at least have proof of
identity
p. If in doubt about -1.0. presented, ask your
supervisor.

a. Parent may present acceptable I.D. for a
minor.
b. Parent's library card on file.
c. Any of the previously listed adult I.D.s
are acceptable.
d. Card may be issued on incomplete 1.0., but
set card to STATUS Il~U and patron must
bring acceptable I.D. next time in.

a. Business cards
b. Unofficial letters or documents
c. Phone book or city directory listings.

LIBRARY CARD PROCEDURE

10. TOURISTS AND GUESTS .
Tourists and guests residing at.' properties w'1thin the city
limits may be issued a temporary library card.
Ask for proof of residency.
A. Receipt, rental agreement, or lease that shows the
person's name and the place of residency.
B. Verbal or written assurance from a city resident that the
person is a guest. (Preferably he/she would check out the
guest's books on the resident's card.)
c. Written notice from person in charge of a city property
. that the person is a guest.
Ask for identification at the alternate address.
Set expiration date for a few days after they expect to
leave, or 6 months.
NOTE: You may accept any reasonable proof in extreme cases
i.e., a family with only a motel key.
