Chairman Dunn has spoken to Frederick Woodard, Firefighter, and his doctor concerning
his ~isability pension. The proper letters have still not been received.

John Nicholson questioned the latest letter submitted by Mr. Woodard. This letter does not
indicate the extent of disability. Michael Stuart requested a letter be sent to Mr. Woodard
advising him of the next. date and asking him to attend this meeting in an effon to resolve

Chairman Dunn submitted a letter from Dr. Ferdcrigos regarding John Ferzola. Firefighter.
This lelter states since there is no light duty for firenghters. Dr. Ferderigos has detcnnined
that John FerzolB is disabled as a firefighter.
Dr. Ferderlgos also submitted a bill for S2S.00 for dictation and transcription. This bill will
be paid out of the Pension Fund in this Instance. In the future. a standardized letter will be
sent to all doctors stating what infonnation should be contained in the response.
Secretary Laursen requested the Committee use the Conn at submitted by him when making
motions so the effective date of the pension and any other pertinent information will be
reflected in the minutes.
Jobn Nicholson Jl)oved that John Ferzola be granted a job-connected disability pension. to
be effective when aU sick leave and worker's compensation benefits have expired. based on
John Ferzola's disability tbat resulted from on-the-job circumstances and which is
described as a herniated disk. said disability making him permanently unable to pcrfonn
his jOb and which is, documented by medical statements of Dr. Fred Fcrderigos. dated
12-23-87; Dr. Edward Feldman. dated 8-28-87; and Dr. A. Schneider. dated 9-29-87. The motion
was duly seconded by Michael Stuart and carried unanimously.
The. Committee requested that in the future a copy of the "no smoking" agreement form for
Police. Officers and Firefighters be attached to the letter approving them for pension.
Secretary Laursen stated for the record he will write to the Police and Fire Chiefs at the
request of the Committee for infonnation as to the method in which this agreement form is
prcseiued 'to, the. employees. The Committee wants to make sure the employees are aware of
this restriction and understand the con~equences if it is violated.
The meeting adjourned at 9:58 A.M.
