Chairman Dunn called meeting to order.
Chairman Dunn called off the names of the following new employees to be approved
for membet;ship' in the pension fund:
James A. Johnson, WWTP Operator B

~ Committee approved the above employees unanimously.
City Nurse will hold physical form on John Rich. She is to check his blood pressure
several times a week and if blood pressure is okay, Committee will approve him for
pensionat'the next PAC meeting.
Committee ,rejected Geroge Anderson, Building & Maintenance Division, for membership in
the ,pension fund at the recommendation of the Walk-In Clinic.
Chairman advised the Committee he had n request for retirement pension from Captain
James 'O'Neill, Police Department. Captain O'Neill has completed twenty years with
. the P~lice Department and is eligible for pension. Committee unanimously approved
Captain O'Neill for Retirement Pension under the employees' Pension Fund.
Chairman Dunn read the new "Contribution Release Formll that will be used in the future
fot; all employees to sign when they are resigning from employment with the City. The
form was designed by Carole Greiner in the Payroll Division and the City Attorney's
Office. Committee approved the new form unanimously.
Chairman Dunn recommended ~hat' a'Payroll,emp1oyee advise a terminated employee as to
the meaning of'the form.
Chairman Dunn asked Secretary Laursen to write a letter to the City Manager's Office
. asking when the, Pension Re-write Committee will start holding meetings.
