Chairman Dunn called meeting to order.
Committee discuBsed the physicals on employees who had been hired. After looking
ov'er the physical forms, the Committee agreed that the form was inadequate.
David K~ieger moved that the present form was not acceptable. Seconded by
John Nicholson and motion carried.
City Nurse Nancy Degner presented to the Committee a form that she had designed and
the Committee'recommended that it be printed for future use by the Walk-in Clinic.
Mr. Laursen advised '"the' Conunittee that if they would recommend the form be used for
pension phYSicals, he would take it to Mr. Weimer and see if he would approve the
form. ,Mr. Laursen suggested the regular form (formerly used) be sent to those
employees already processed by the Walk-In Clinic, have them fill out the medical
, historY portion and return to the City Nurse who would send them to the Walk-In
Clinic.' The Walk-In Clinic can then copy the information from their form on to the
,City 'form. In'the meantime, until the new form is printed, other new employees will
, use the old form to take to the Walk-In Clinic.
Committee will vote on accepting the following employee~ into the Pension Plan at the
,next meeting:

Keith Ramon
Carole Holschuk
Harmon Stuls
Glenn Daniel
Mark Perrigone
Leroy Robinson
Chairman Dunn 'asked Personnel Director to check with Carole Greiner as to the status
of the form "Return to Contribution 'Release." "
Chairman Dunn stated he had talked to Sergeant O'Neill's daughter and':asked her to have
her father'come and sign the necessary papers for his retirement pension.

City Nurse would like copies of letters from the three doctors regarding
Alan Kronschnabl.
Chairman Dunn advised the Personnel Director that he had received a letter from
the City Manager regarding the Pension Re-write Committee. He also received a
letter froR,Mr. ,Weimer regarding the' Committee and also stating that John Nicholson
could be a member' of the, Committee. '
There being no further business John Nicholson moved the meeting be adjourned.
Seconded by David Krieger and motion carried.
