Present: Ray Kaminakna, Chnirman Terry Clements, Member Absent: Claude Zukowski, Member Joyce Bayerle, City Nurse City Representative: Carole Greiner, Finance Department Terry Clements moved the Minutes of meeting of December 2, 1982 be accepted. Seconded by Ray Kaminskas and motion carried. Chairman Kaminskas presented letter from Doris McLendon, widow of He~bert McLendon, requesting widow's pension. Terry Clements moved that the Committee approve . Herbert McLendon for a non-job connected disability pension and that his widow Doris McLendon be approved for widow's pension in accordance with the rules of the pension plan. (Finance Department to figure amount of widow's pension.) Motion seconded by Ray Kaminskas and carried. Terry Clements moved, Ray Kaminskas seconded the motion and it carried that the ,following employees be accepted into the pension plan. Clements moved that the Pension Advisory Committee approve two forms drawn up by the Finance Department for retirees under pension. 'l'he first form is a "Certification of Pension Entitlement" form that is to be filled in. each year prior to April 1 and notarized. If this form is not returned by Apr,il I.' benefits will be withheld until the form is returned. . The: other form advises retirees 'that the Pension Advisory Conunittee's Board .of Trustees is currently working on re-examination physicals on an annual basis for disability pensioners.
