This InSite - Florida Intelligence System Agency Agreement is made and entered into this 3o`h day of may, 2008
,by and between the Clearwater Police De artment,
(Agency Name)
(hereinafter referred to as the Participating Agency) and the Florida Department of Law Enforcement,
(hereinafter referred to as POLE). The Integrated Florida Intelligence: System Operating Guidelines set forth the
conditions g'ov'erning the Participating Agency's involvement and responsibilities for the use and dissemination of
information transmitted to, or retrieved from the system. The Operating Guidelines are available for review on-line
at http:i/osiweb.fdle.flejn.net/InSite/inSite.htm.
Implementation of this Agency Agreement is i.ntcnded to enhance and foster the exchange of criminal intelligence
among the criminal justice community through the use of the Florida Intelligence System maintained by FDLF..
f'or, and in consideration of, the mutual promises and covenants contained herein, the sufficiency of which is hereby
acknowledged, the parties hereto agree to the responsibilities set forth for the Participating Agency pertaining to
audits, security, responsibility for information, decutnentation and dissemination of information, liability for
dissemination, disclaimer of responsibility for accuracy of records and training contained in the Florida Intelligence
System Operating Guidelines.

agency head of the participating Agency has read and agrees to follow and adhere to all the rules, regulations and
