BACKGROUND: in response to a requirement to provide annual physical examinations to approximately 340 police and fare personnel, sealed bids were solicited from two (2) private physicians and six (6) medical facilities engaged in performing such examinations. Bids were opened on October 28, 1980. Two (2) bids were received and submitted to the staff for evaluation. During the staff evaluation it was discovered that both bidders had not given adequate consideration to scheduling shift workers plus the X-Ray and R.P.R. test requirements were omitted from their bid responses. Both bids were, therefore, declared non-responsive to bid requirements. Specifications were re-drafted by the staff and bads were again solicited from all bidders on the original bid list. aids were opened on Decenber 2, 1980 and the results are attached. Police and Fire Department staff personnel reviewed all bids and recomwnded award to the low bidder, Clearwater Coatmunity Hospital. This year the physical exams will be given in a fixed facility for the first time. Last year a mobile facility, located at the Main Fire Station, was used. While the mobile facility offers greater scheduling flexibility for shift workers--minimizing overtime costs to the City--it does not provide for a physician on-site to administer the examinations. Last year some police and fire personnel expressed concern over the lack of an on-site physician. However, since the City must pay overtime to police and fire personnel who are required to get an annual physical on their off-duty time, overtime costs cannot be ignored when looking at total cost (bid cost & overtime cost) to the City. The staff feels that the use of. Clearwater Community Hospital this year will provide conrgarison data for future decisions involving the use of fixed versus mobile facilities for future examinations. BACKGROUND: In response to a requirement to re-cover furniture in city Administration Offices, sealed bads were solicited from fifteen (15) inter- ior decorator and upholstery firms. Bids were opened on October 16, 1980 and the results-are attached. Godwin Upholstery submitted a bid for materia which was unrealistically low. The substantial difference between his bid and other bids led the Put chasing officer to believe that an error may have been made. He contacted Godwin Upholstery which verified that a clerical error had been made in computation and transposition of prices. After coun- sel with the Chief Assistant City Attorney, the Purchasing Officer provided Godwin Upholstery an opportunity to withdraw its bid for materials, On December 10, 1980 Godwin Upholstery formally withdrew its material bid. The bid terms and conditions provide that the City may award a contract in whole or in part, whichever results in the lowest total cost to the City and is otherwise in its best interest. Therefore, Godwin Upholstery was neither requested nor required to withdraw its bid for labor. Godwin Upholstery appears to have the capability to perform the labor portion of this bid with no problem and has expressed a sincere desire to provide labor only. The fabric material will be purchased from Interior Architects Ltd., the next low bidder, and furnished to Godwin Upholstery. All interested accounting firms have been invited to submit proposals. Eleven firms proposed. These have been reviewed by a staff committee. Accounting firms are not permitted by law to submit bids. It is necessary for the City to select a firm based on qualifications and then to negotiate a satisfactory fee. If the negotiations are not successful, then the first ranked firm is dismissed and negotiations connnence with the second ranked firm. BACKGROUND: The City Commission, during the. 1980181 Budget hearings, were made aware of maintenance problems at Glen Oaks and expreased an interest in staff securing a qualified golf course person with expertise in correcting the situation. To accomplish this, a reorgani- zation of the golf course was effected which resulted in the following: 1. Organizationally, Glen Oaks was removed from the Recreation Division and made a separate "Agency" within the Parks and Recreation Department. 2. An appointed position of Golf Course Superintendent was created which took the place of the former classified position of Golf Course Manager. 3. Mr. Chick Lewis, was appointed to the new Superintendents position. He had previously been with the City and returned after having been in charge of golf course operations at Bast Lake Woodlands for the past six years. Mr. Lewis is extremely knowledgeable in all phases of golf course operations, has many years of experience and is respected by staff and golfing participants alike. 4. The reorganization actually resulted in a savings of approximately $1,275. to the Per- sonnel Budget of Glen Oaks. However, the system at Glen Oaks is man- 1 and we have always had to have personnel report to work early in the morning to water ahead f the golfers. This system has been ineffective (please see attached memo from Hr. Lewis) and we request that an additional employee be authorized so that an evening watering schedule could be instituted. This person would perform other maintenance duties besides just watering and his afternoon and evening hours would benefit the course in other ways as pointed out in Mr. Lewists memo. The additional requested position has a total annual cost of $10,849.34, but the budget im- pact for the remainder of this fiscal year will only be $6,236.04. BACKGROUND: Construction of the North Jetty is to stabilize Clearwater Pass Inlet and the public beach to the north as part of the Clearwater Pass Bridge protection project. Funding is provided by Bond Revenue obtained as part of the total bridge project. The initial bid for the 800 ft. long, Jetty represents the maximum length specified by contract. The 700 ft, length, with the price shown, is the City's option to build a shorter one. ('phis is provided for in the contract which states: "...as total funds available for the project are fixed by the City, the length will be set for a 100 ft. increment -- not less than 400 linear ft. nor more than 800 linear ft., based on bid unit cost of materials.) The Mobile Home Park Board was established to administer the provisions in the Ordinance. The purpose of the proposed amendment is as follows: - To revise the guidelines for mediation and arbitration procedures, - To clarify definitions. - To provide guidelines for individual negotiations with management. - To provide for'representation by negotiating agents and negotiating units and for their election. The Mobile Home Park Board held a public meeting on'Qctober 8, 1980 to discuss the proposed amendments and obtain public input. The intended use of the property is to establish a congregate living facility similar in operation to the "Share-A-home" facility approved by the City Commission on property east of the subject property. Plans are to provide accomodations in the existing dwelling unit for 14 elderly persons with above average incomes desiring a group living arrangement. In order to establish a congregate living facility at this location, it is necessary to amend the original RPD plan. The Board stated that they agreed with the concept of congregate living for the elderly,but were concerned with the following: 1. Availability of sufficient parking space 2. I BACKGROUND: If approved, this project will construct sidewalk along the South side of S.R. 580 from the City property, which contains the water storage tanks, East to the church property adjacent to Landmark Drive. Ad- joining property is owned by the Order of Ahepa. This sidewalk will provide a connecting link between existing sidewalks lead- ing from Winding Wood Subdivision to the Leila Davis Elementary School. This project was initiated as a response to a request from the Leila Davis Safety Committee, Chairman Ed Mazur, Jr. After proper notices, a Preliminary Hearing was held at 2:PM on October 29, 1980. The one owner involved, Order of Ahepa, did not appear at the Pre- liminary Hearing. The Commission Meeting today is a Public Hearing for this project. Keith Crawford, Traffic Engineer and Max Battle, Public Works Director both concur with this recommendation. The estimated assessment 'for this project is $2.25 per square foot of sidewalk area.
