The meeting was called to order at 9:05 a.m. by Chairman
Polatty in the Operations Conference Room.
Nick Lewis moved to approve the minutes of the meeting of
August 10, 1989. The motion was seconded by Bob Maran and
carried unanimously.

ITEM '1 - CLEARWATER COMMUNITY HOSPITAL located at the
southeast corner of the intersection of Druid Road and
Highland Avenue
RECERTIFICATION OF FINAL SITE PLAN - Approved with Conditions

Pertinent comments from each department representative are
incorporated into the motion listed below. Attachment'1 is
a part of this item. There were no representatives present
from Clearwater Community Hospital.
MOTIONs Sandy Glatthorn moved to approve the revalidation
of the Final Site Plan, based upon a finding of this
Committee that the plan conforms with the design guidelines
for site plan review subject to the conditions of December 6,
1988, certified plan with amended No. 6 to read as follows:
The requisite initial building permit must be procured
prior to February 24, 1990, and all Certificates of
Occupancy must be obtained prior to February 24, 1993.
The motion was seconded by Vie Chodora, there was no
discussion, and the motion carried unanimously.

MOTION: Sandy Glatthorn moved to approve the final site
plan based upon a finding of this Committee that the plan
conforms with the design guidelines for site plan review
subject to the following:
Prior to certification copies of D.E.R. and S.W.F.W.M.D.
permit applications must be provided to the Public Works
Department, and copies of the approved permits are to be
provided prior to the issuance of a building permit.
Prior to certification the outlet control structure must
be relocated to the southeast corner of the detention
pond, as required by the Public Works Department.

The vehicular areas shall be constructed in accordance
with City standards, i.e. paving, curbs, etc.
The applicant shall meet with Traffic Engineering
regarding traffic flow, related signage, and site plan
shall be revised as required by the Traffic Engineer
prior to certification.
Prior to the issuance of a Certificate of Occupan~,
detector check valves for the sprinkler system will be
installed by the City of Clearwater.
The siamese connection that is required by the Fire
Marshal must be shown on the plans prior to
certification.

The option to pipe the ditch along the east side and
redesign the vehicular areas should be discussed with
Public Works Department prior to certification.
Signs and fencing walls are subject to separate review
and permitting processes.
,Site data must be revised to included minimum/maximum
requirement prior to certification of the site plan.
The requisite initial building permits shall be procured
within six months from certification of the site plan,
and requisite Certificates of Occupancy shall be
obtained within three years from certification of the
site plan.
Motion was seconded by Bob Maran, and carried unanimously.

ITEM 13 - SAND CASTLE (EXPANSION/RENOVATION) located at the
northwest corner of Osceola Avenue and Drew Street
FINAL SITE PLAN

Mr. Graham presented new plans to the Committee that show
some of the changes that had been discussed previously. The
changes are I (1) there is a new fire hydrant located at the
northeast entry drive; (2) the existing water line is shown
with the water for the new building attached to that existing
line; (3) a gas meter is shown as a new location; (4) the
retention pond is shown on the plan; and (5) the parking
areas are now readjusted to show how they exist, as the
variance was turned down.

of right-of-way for Osceola Ave. and 25 ft. of drainage,
utility, and sidewalk easements are to be dedicated to
the City of Clearwater. An Affidavit of No Lien is to be provided
with the dedication as is required by the City Attorney.

AMENDMENT TO MOTIONs Sandy Glatthorn moved to amend the
original motion, for condition No. 2 to reads

There was no further discussion on the original motion, and
the motion carried unanimously.
Mr. Graham presented a letter that he had prepared regarding
the classrooms, or course rooms, which the City Commission
had'questioned.
The meeting adjourned at approximately 10:05 a.m.
