4. A 10 foot utility easement is required adjacent to the Betty Lane right-of-way;
5. The applicant must relocate the parking space immediately adjacent to the dumpster
location in order to provide 'adequate access to the dumpstp.r; ,

7. The Landscape Plan must be revised to show a new location for the tree proposed to

the following:

2. Additional fire hydrants should be shown on the final site plan as required by
the Fire Marshall;
3. The detention pond should be designed as a dry'detention area as required by

shown on the site plan;

complete the top portion of the attached work sheet and return
same to the Parks and Recration Department. A copy of the
most recent purchase contract and a copy of the just value
from the Pinellas County Property Appraiser's Office ha~cal-
ready been received.
The open space and recr.eation facility land assessment fees
will be payable prior to City certification of final site plan.

Engineer at 462-6790 to coordinate meter sizing.
Failure to do so will delay permit processing at
the ~uilding Department.
. Depending an the water meter size, this site may
require an above-ground meter installation.
. A Reduced Pressure Backflow Prevention Device
is ~equired on the cuatomer's side of the water
met.er.
. Additional fire hydrants may be required by the
Fire Department. Allan-site hydrant locations
will require a 10' utility easement over the
fire line up to and including the hydrant prior
to the issua~ce of a Building permit.
. A Pinellas County Health Department Permit is
required prior to the installation of the water
service and any fire lines that are required.

COMMENTS: Site Plan - Although this Diagnostic Center is being
constructed on property for which an open space assessment fee
has already been paid, an additional parcel of land is being
added to the site plan which will accolTunodate parking for the
Diagnostic Center and the M.R.I. Facility. Therefore, an open
space assessment fee will be required to be paid for that
portion of land ~ot included in the original site plan.
The developer or his representative should complete the top
portion of the attached work sheet and return same to the Parks
and Recreation Department. Copies of the just values and purchase
price of the additional sites should be attached to the work
sheet.
This open space fee will be payable pr~or to City certification.
