As Trustees of the City of Clearwater Employees' Pension Fund, you are hereby notified that the
employee(s) listed below have been duly examined by a local physician and each has been designated as a
"first class risk". The employment of these individuals brings the number of FTE's as of this date to
1770.76 of 1844.9 budgeted positions (including the City Commission).
These employees are eligible for pension membership as noted in the Pension Eligibility Date column
below, and it is the recommendation of the Pension Advisory Committee that they be accepted into
membership.

Department: Douglas J. Wlasiuk, Fleet Mechanic, Solid Waste/General Services; Carol A. Turcotte,
Property Supervisor, Police Department; and Charles E. Watson, Recreation Programmer, Parks &
Recreation Department, be granted regular pensions under Section(s) 2.393 and 2.397 of the
Employees' Pension Plan as approved by the Pension Advisory Committee.

SUMMARY:
Joseph P. Egger, Police Officer, Police Department, was employed by the City on August 17, 1983,
and his pension service credit is effective on that date. His pension will be effective September 1,
2003. Based on an average salary of approximately $46,470 per year over the past five years, the
formula for computing regular pensions, and Mr. Egger's selection of the 100% Joint & Survivor
Annuity, this pension will be approximately $25,089 annually.
Walter A. Yasensky, Fire Fighter, Fire Department, was employed by the City on January 1, 1974, and
his pension service credit is effective on that date. His pension will be effective July 1, 2003. Based
on an average salary of approximately $51,903 per year over the past five years, the formula for
computing regular pensions, and Mr. Yosensky's selection of the Joint & Survivor Annuity, this
pension will be approximately $42,039 annually.
Wayne R. Meissner, Police Captain, Police Department, was employed by the City on December 8,
1969, and his pension service credit is effective on that date. His pension will be effective July 1,
2003. Based on an average salary of approximately $72,423 per year over the past five years, the
formula for computing regular pensions, and Mr. Meissner's selection of the 75% Joint & Survivor
Annuity, this pension will be approximately $67,426 annually.

Matthew Moore, Librarian, Library, was employed by the City on February 9, 1984, and his
pension service credit is effective on that date. His pension will be effective March 1, 2004.
Based on an average salary of approximately $49,276 per year over the past five years, the
formula for computing regular pensions, and Mr. Moore's selection of the 100% Joint & Survivor
Annuity, this pension will be approximately $26,432 annually.
Douglas J. Wlasiuk, Fleet Mechanic, Solid Waste/General Services, was employed by the City
on April 17, 1978, and his pension service credit is effective on that date. His pension will be
effective July 1, 2003. Based on an average salary of approximately $38,439 per year over the
post five years, the formula for computing regular pensions, and Mr. Wlasiuk's selection of the
10 Year Certain & Life Annuity, this pension will be approximately $26,188 annually.
Carol A. Turcotte, Property Supervisor, Police Department, was employed by the City on
September 10, 1973, and her pension service credit is effective on that date. Her pension will
be effective October 1, 2003. Based on an average salary of approximately $37,156 per year
over the past five years, the formula for computing regular pensions, and Ms. Turcotte's
selection of the 100% Joint and Survivor Annuity, this pension will be approximately $30,321
annually.
Charles E. Watson, Recreation Programmer, Parks & Recreation, was employed by the City on
September 6, 1978, and his pension service credit is effective on that date. His pension will be
effective November 1, 2003. Based on an average salary of approximately $35,018 per year
over the past five years, the formula for computing regular pensions, and Mr. Watson's
selection of the Joint & Survivor Annuity, this pension will be approximately $24,222 annually.
The Pension Advisory Committee approved these pensions on July 14, 2003. Section 2.393 (p)
provides for normal retirement eligibility when a participant has completed twenty years of
credited service in a type of employment described as "hazardous duty" and further
specifically defines service as a Police Officer, Firefighter, and Police Captain as meeting the
hazardous duty criteria. Section 2.393 also (p) provides for normal retirement eligibility when a
participant has reached age 55 and completed twenty years of credited service, has
completed thirty years of credited service, or has reached age 65 and completed ten years
of credited service. Mr. Yasensky, Mr. Meissner, and Mr. Egger qualify under the hazardous
duty criteria. Ms. Turcotte qualifies under the 30 years of service criteria. Mr. Moore, Mr.
Wlasiuk and Mr. Watson qualify under the 20 years of service and age 55 criteria.

and 2.398 of the Employees' Pension Plan as approved by the Pension Advisory Committee.

SUMMARY:
Glenn Perry, Parks Service Supervisor I, Parks & Recreation Department, was employed by the City on
August 29, 1973, and began participating in the Pension Plan on March 1, 1974. Mr. Perry terminated
from City employment on May 2, 2003.
The Employees' Pension Plan provides that should an employee cease to be an employee of the City
of Clearwater after completing ten or more years of creditable service (pension participation), then
such employee shall acquire a vested interest in the retirement benefits. Vested pension payments
commence on the first of the month following the month in which the employee normally would
have been eligible for retirement.
Section 2.393 (p) provides for normal retirement eligibility when a participant has completed twenty
years of credited service in a type of employment described as "hazardous duty" and further
specifically defines service as a Police Officer as meeting the hazardous duty criteria. Section 2.393
also (p) provides for normal retirement eligibility when a participant has reached age 55 and
completed twenty years of credited service, has completed 30 years of credited service, or has
reached age 65 and completed ten years of credited service. Mr. Perry would have completed
thirty years of service on March 1, 2004. His pension will be effective April 1, 2004. This pension was
approved by the Pension Advisory Committee on June 3, 2003.
