As Trustees of the City of Ciearwater Employees' Pension Fund, you are hereby notified that the
employee(s) listed below have been duly examined by a locai physician and each has been designated as a
"first class risk". The emptoyment of these individuals brings the number of FTE's as of this date to
1683.02 of 1799.3 budgeted positions (including the City Commission).
These employees are eligible for pension membership as noted in the Pension Eligibility Date column
below, and it is the recommendation of the Pension Advisory Committee that they be accepted into
membership.

and her pension service credit is effective on that date. Her pension will be effective
Based on an average salary of approximately $47,499 per year over the past five years, the (ormula for computing
rebular pensions, and Ms. )ewett's selection of the 100% Joint S Survivor Annuity, this pension will approximate
$28,083 annually.
Nancy Burgarella, Police Officer, Police Department, was employed by the Cih� on October 18,1971, and lier
pension service credit is effective on April 18, 1972. Her pension will be effective
September 1, 2001.
Based on an average salary of approximately $47,6G0 per year over tl�e past five years, tl�e formula for computing
regular pensions, and Ms. Burgarella's selection of the 100% )oint & Survivor Annuity, this pension �vill
approximate $37,979 annually.
Wilson McGill, Jr., Parks Supervisor I, Parks & Recreation Department, was employed by the Ciry on
)une 23, 1967, and I�is pension service credit is effective on tl�at date. His pension will be effective
)uly �, 200�.
Qased on an average salary of approximately $43,647 per year over the past five years, the formula for computing
regular pensions, and Mr. McGill's selection of the 75% Joint & Survivor Annuity, tl�is pension will approximate
$39,488 annually.
These pensions were approved by the Pension Advisory Committee on April 12, 2001. Section 2.393 (p)
provides for normal retirement eligibility when a participant has completed twenty years of credited service in a
rype of employment described as "hazardous dury" and further specifically defines service as a Police Officer as
meeting the hazardous duty criteria. Section 2.393 (p) also provides for normal retirement eligibility when a
participant has reached age 55 and completed twenty years of credited service, has completed 30 years of
credited service, or has reached age 65 and completed ten years of credited service. Ms. )ewett and
Ms. Burgarella qualify under the hazardous duty criteria. Mr. McGill qualifies under the age 55 and 20 years of
service and 30 years of service criteria.

Employees' Pension Plan as approved by the Pension Advisory Committee.

SUMMARY:
Patricia Schrembs, Police Service Technician, Police Department, �vas employed by tl�e City ori January 28,
1991, and her pension service credit is effective on that date. Sfie terminated from City employment on
March 30, 2001.
The Employees' Pension Plan provides that should an employ�e cease to be an employee of the City o� Clearwater
after completing ten or more years of creditable service (pension participation), then sucli employee shall acquire a
vested interest in the retirement benefits. Vested pension payments commence on tl�e first of tlie month following
the month in which the empfoyee normally �vould have been eligit�le for retirement.
Section 2.393 (p) provides for normal retirement eligibility when a participant has reached age 55 and completed
twenty years of credited service, has completed 30 years of credited sen�ice, or has reached age 65 and completed
ten years of credited service. Ms. Schrembs �vould liave completed ten years of service and reached a�e G5 on
Marcli 17, 2003. Her pension �vill be effective on April 1, 2003. Tiiis request was approved by tl�e Pension
Advisory Committee on April 12, 2001.
