Based on an average salary or approximately $42,081 per year over the past five years, the forrnula for computing
regular pensions, and Mr. Flarding's selection of the Life Annuity, this pension will approximate $29,151
annuaily.
iDavid �oster� Gas Sp�cialist, Gas l�epartment, was employed by the City on
4ctober 11, 1976, �nd his perision service credit is effective on April 11, 1977. His pension will be effective
�pri) 1, 2000.
Based on an average salary of approximately $37,873 per year over the past five years, the formula for computing
regular pensions, and Mr. Foster's selection of the 50% Joint & Survivor Annuity, this pension will approximate
$25,270 arinually.
Joseph Johnson, rourier, tnfc�rmation Technology Department, was emplvyed by the City on
March 7 E�, 1970, and his pension service credit is effective on September 16, 1970. His pension will be effective

Based on an average salary of approximately $2�,965 per year over the past five years, the formula for
computin� regular pensions, and Mr. )ohnson's selection of the Life Annuity, this pension will approximate
$20,625 annually.
These pensions were approved by the Pension Advisory Committee on June 8, 2000. Section 2.393 (p)
provides for normal retirement eligibility when a participant has reaclied age 55 and completed twenty
years of credited service, has completed 30 years of credited service, or has reached age 65 and completed
ten years of credited service. Mr. Dussault, Mr. Harding, and Mr. Foster qualify under the age 55 and 20
years of service criteria and Mr. J�hnson qualifies under the 30 years of service criteria.

SUMMARY:
G. Ann Gilsdorf, Compute�• Aided Drafting Technician, Public Works Department, was employed by the City on .
March 12, 1984, and her pension service credit is effective on that date. She resigned from City employment on
June 2, 2000.
T�ie Employees' Pension Plan provides that s�ou(d an employee cease to be an ernployee of the Ciry of Clearwater
after campleting ten or more year� of creditable service (pe��sion participation), then such employee shall acquire a
vested inierest in the retirement benefits. Vested pension payments commence on the first of the r�onth following
the month in whic�i the employee norrnally would have been eligible for retirement.
Section 2.393 provides for normal retirement eli�ibility when a par�icipant has completed twenty years of credited
service and reached age 55.
Ms. Gilsdorf would have cornpleted twenty years of service and reached age 55 on tvdarch 12, 2004. Her pension
will be effective on Apri) 1, 200�. This request was approved by the Pension Advisory Committee on June 8,
2000.

SUMMARY:
Gerald Ermlich, General Support 5ervices Department, was employed by the City on August 10, 1987, and his
pension service credit is effective on that date. Mr. Ermlich passed away on May 22, Z000. Patricia Ermlich,
widow of Gerald Ermlich, applied for death benefits on May 30, 2000. Mr. and Mrs. Ermlich were legally married
on December 31, 1975, thereby entitl ing her to d�ath benefits under provisions of the Ernployees' Pension Plan.
Mrs. Ermii�h's request for death benefits was approved by the Pension Advisory Committee on )une 8, 2000. The
amount of IvSrs. Errnlich's pension v�ill be computed by the Finance Department at a(ater date. This pension will
be effective june 1, 200C.
Based on an average salary of approxirnately $35,225 per year over filie past five years, this pension wifl
approximate $12,386 ar�nually.

Terrance B. Griggley, a City of Clearwater employee, died on June 17, 2000. Mr.
Griggley did not have a Will and there have been numerous requests for release of
pension moneys, wages and benefits by various family members.
1 am requesting your approval to file �n interpleader action in circuit court to determine the
amount and payee for such pension fund moneys.
