This letter is to acknowledge receipt of your application dated
06/13/79 for renewal of your National Pollutant Discharge Elim-
ination System (NPDES) permit. A check in the amount of $ N/A
was also received.

Your renewal application will carry the same NPDES number F~0021857
as was previously assigned to this facility. The application will
be processed and the permit reissued in accordance with established
priorities. However, there may possibly be a delay in the reissuance
due to an unusually heavy workload caused by the large number of per-
mits expiring during this same period. Except in unusual circumstances,
if there is a delay beyond the expiration of your existing permit, you
are still authorized to discharge pursuant to 5 U.S.C. Section 558(c)
of the Administrative Procedures Act. Therefore, unless we notify you
in writing to the contrary, your present permit will remain fully
effective and enforceable pending issuance of your new permit.

for administrative hearing. This petition must be filed within
fourteen ,(14) days of the receipt of this letter. Further, the
petition must conform to the requirements of Section 28-5.15,
Florida Administrative Code, (copy enclosed). The petition must be
filed with the Office of General Counsel, Department of Environmental
Regulation, Twin Towers Office Building, 2600 Blair Stone Road,
Tallahassee, Florida 32301.
If no petition is filed within the prescribed time, you will be
deemed to have accepted this permit and vlaived your right to request,
an administrative hearing on this matter.
Acceptance of the permit constitutes notice and agreement that the
department will periodically review this permit for compliance,
including site inspections where applicable, and may initiate enforce-
ment action for violation of tli0 conditions and requirements thereof.

DECISIONS DETEru~INING SUBSTANTIAL INTERESTS
28-5.15 Requests for Formal and Informal Proceedings
(1) Requests for proceedings shall b~ made by petition to the
agency involved. Each petiti.on shall be printed, type-
written or otheniise duplicated in legible form on \vhite
paper of standard legal size. Unless printed, the impression
shall be on one side of the paper only and lines shall be
double spaced and indented.
(2) All petitions ,filed under these rules should contain:
(a) The name and address of each agency affected and
each agency I s file or identification number, if knm.m;
(b) The name and address of the petitioner or petitioners;
(c) All dispu~ed issues of material fact.
