This permit will expire on 12/30/78, and will be subject to the
conditions, requirements and restrictions checked or othcr~ise
indicated in the attached sheet "Construction/~ Permit

This permit is issued under the authority of Florida Statute
403.061(16). The time limits imposed herein are a condition to
this permit and are enforceable under Florida Statute 403.161.
You are placed on Notice that the Department ~i1l review this
permit before the scheduled date of expiry and will seek court
action for violation of the conditions and requirements of this
permit.
You have ten days from Lhe date of receipt hereof within which
to seek a review of the conditions and requirements contained in
this permit. Failure to file a written request to review or
modify the conditions or requirements contained in this permit
shall be deemed a waiver of any objections thereto.

(X) 1. Construction of this installation shall be completed by 6/30/78
(X) 2. This construction permit expires on 12/30/78 ,following an
initial period of operation for appropriate testing to determine
compliance with the Rules of the Florida Department of Environ-
mental Regulation.
(X) 3. All applicable rules of the Department including design discharge
limitations specified in the application shall be adhered to. The
permit holder shall also comply with county, municipal, federal
or other state regulations prior to construction.
(X) 4. The applicant shall continue the retention of the engineer of
record for the inspection of the construction of this project.
Upon completion the engineer shall inspect for conformity to
construction permit applications and associated documents. A
report of such inspection shall be submitted by the engineer
to the Department of Environmental Regulation for consideration
toward the issuance of an operation permit.
(X) 5. This construction permit is issued with the understanding that
there will be no discharge to area surface waters and that all
plant effluent will be retained on the owner's property~ (or to
deep well at a site to be determined).
() 6. The evaporation-percolation/polishing pond shall be enlarged
as necessary and sufficiently maintained to insure against
discharge to area surface waters. Two shallow sampling wells
will be installed located in the watertable, hydraulically
downstream from the pond (ref. Chapter 17-19.08 FAC).
eX) 7. The STP's treatment process shall include design features which
will result in total compliance with Chapter 17-3 of the Florida
Administrative Code, which requires 90% or better reduction in
BOD (5-day) and Suspended Solids.
(X) 8. The chlorine residual in the effluent must be maintained at
(~xix~~~ (1.0 ppm) minimum, after 15 minutes contact, at
maximum flow.
(X) 9. If found to be necessary for protecting downstream/receiving
waters, tertiary treatment for nutrient removal and/or additional
BOD reduction shall be provided.

(x) 10. This construction permit is issued with the understanding that
there are no water supply wells closer than 100 feet from the
effluent disposal area which would be endangered.
(X) 11. Construction for expansion to a larger capacity shall not be
initiated prior to application to and permitted by the DER.
( ) 12. When an areawide district/metropolitan collection and treatment
system becomes available, the DER will require the permittee to
connect the source to such a system within a specified period of
time. It will then be incumbent upon the permittee to comply
with such requirements, cease operation of the permitted source
and remove the permitted plant from its site.
(X) 13. Before an operation permit is issued, the applicant shall submit
evidence that an operator certified under Chapter 17-16 Florida
Administrative Code is retained for day-to-day maintenance and
operation of the treatment facilities. This plant will be
classified as a Level II, Class A plant. As such, it will
require that a Class x-operator be-on site 24 hours/day, 7

DESCRIPTION OF PROPOSED PROJECT

The Clearwater Northeast Wastewater Treatment Facility will provide capacity to process
an avera!:le of 8 MGD of raw wastewater to the required de!:lree of secondary treatment.
The plant wi II include mechanical screenin!:l, aerated !:lrit removal, odor control,
primary clarification. conventional activated sludge. secondary clarification and
disinfection processes. Sludges.will be digested aerobically and beneficially applied
to the land in a controlled manDer for fertilization and soil conditionin9 Durposes.
The project will provide the required dearee of treatment preparatorv to effluent
disposal by deep well injection; the plannina. desian and construction of which will
roceed concurrentl with the construction of the subject treatment system.

Dear Mr. Macfarlane:
We are in receipt of your request for an extension of the Temporary
Operation Permit for the subject project.
Based on the information submitted, we are hereby extending the
expiration date of the Temporary Operation Permit No. TP 52-2340
from April 1, 1976 to December 30, 1978.
A permit has been issued to the City of Clearwater to expand their
present 1.0 MGD facility to 8.0 MGD. Construction completion is
scheduled for June 30, 1978 with permit expiration set at December
30, 1978.
It is recommended that the implementation schedule submitted with
your request be slightly accelerated in order for construction
completion of connecting transmission facilities to coincide with
completion of plant construction and full phase-out of the Safety
Harbor plant to occur by December 30, 1978.
Any time delays applicable to Clearwater's construction program
will be automatically applied to the Safety Harbor schedule.
