In reference to your recent application to this department for approval of the installation of a sewage
collection/transmission system to serve the following project:

Approval of construction only has been granted subject to the following conditions:
1. The design, installation and use of the facilities shall be in accordance with: your applica-
tion dated December 15, 1978 ; the Water Pollution Control Federation
"Manual of Practice No.9"; appropriate provisions of Chapters 403 and 471, Florida
Statutes; and the rules and regulations of the Department of Environmental Regulation.
2. The approval is only for INSTALLATION of a wastewater collection/transmission system
and does not relate to any water supply aspects. The engineer of record shall supervise
the construction of the engineering features of the system. At the end of construction,
but prior to use, the "Certification/Approval to Use" statement (found on Page 2) must
be completed and submitted tot his department and a copy forwarded to the local pro-
gram (if applicable) of your county.
3. This approval is based on the understanding that adequate reserve capacity, as stated in
your application and certified by the trreatment plant owner, will be available at the
Ci ty of Cleatwater NE sewerage treatment plant so that
the wastewaters generated by this project will be collected, treated and disposed of in
compliance with Chapter 403, Florida Statutes and the rules and regulations of this
department.
4. Individual connections that will increase flow to the sewage treatment facility are not in-
cluded in this approval.

departments/agencies may be required.

7. Where water and sewer mains cross with less than eighteen inches (18") vertical
clearance, the sewer will be twenty feet (20' - 0") of either cast iron pipe or concrete
encased vitrified clay pipe, centered on the point of crossing.

should be maintained where practical.

with the application and approved plans. The system, if properly maintained and operated, shall comply

customers have an equal opportunity to obtain such service.
