If no petition is filed within the prescribed time, you will be deemed

Acceptance of the permit constitutes notice and agreement that the
Department-will periodically review this permit for compliance,
including site inspections where applicable, and may initiate
enforcement action for violation of the conditions and requirements

This permit is issued under the provisions of Chapter 403 , Florida Statutes, and Chapter
1 7 -7 , Florida Administrative Code. The above named applicant, hereinafter called Permittee, is hereby authorized to
perform the work or operate the facility shown on the approved drawing(s), plans, documents, and specifications attached hereto and
made a part hereof and specifically described as follows:
Operate a solid waste transfer station serving the City of Clearwater,
approximately 90,000 people. This facility is located at 1005 Old Coachman
Road. It can be further located by being within Section 7, Township 29
South, Range 16 East. The facility accepts between 300-350 tons per day.

GENERAL CONDITIONS:
1. The terms, conditions, requirements, limitations, and restrictions set forth herein are "Permit Conditions:, and as such are bind-
ing upon the permittee and enforceable pursuant to the authority of Section 403.161( 1}. Florida Statutes. Permittee is hereby placed
on notice that the department will review this permit periodically and may initiate court action for any violation of the "Permit Con-
ditions" by the permittee, its agents, employees, servants or representatives.
2, This permit is valid only for the spec.ific processes and operations indicated in the attached drawings or exhibits. Anyunautho-
rized deviation from the approved drawings, exhibits, specifications, or conditions of this permit shall constitute grounds for revoca.
tion and enforcement action by the department.
3, If, for any reason, the perm ittee does not comply with or will be unable to comply with any condition or limitation specified in
this permit, the permittee shall immediately notify and provide the department with the following information: (a) a description of
and cause of non-compliance; and (b) the period of non-compliance, including exact dates and times; or, i1not corrected, the antici-
pated time the non-compliance is expected to continue, and steps being taken to reduce, eliminate, and prevent recurrence of the non.
compliance, The permittee shall be responsible for any and all damages which may result and may be subject to enforcement action by
the department for penalties or revocation of this permit.
4, As provided in subsection 403.087(6), Florida Statutes, the issuance of this perm it does not convey any vested rights or any ex-
clusive privileges. Nor does it authorize any injury to public or private property or any invasion of personal rights, nor any infringe-
ment of federal, state or local laws or regulations.
5. This permit is required to be posted in a conspicuous location at the work site or source during the entire period 01construction
or operation,
6. In accepting this perm it, the permittee understands and agrees that all records, notes, monitoring data and other information re-
lating to the construction or operation of this permitted source, which are submitted to the department, may be used by the depart-
ment as evidence in any enforcement case arising under the Florida Statutes or department rules, except where such use is proscribed
by Section 403.111, F ,S.
7. In the case of an operation permit, permittee agrees to comply with changes in department rules and Florida Statutes after a
reasonable time for compliance, provided, however, the permittee does not waive any other rights granted by Florida Statutes or de;
partment rules.
8. This permit does not relieve the permittee from liability for harm or injury to human health or welfare, animal, plant, or aquatic
life or property and penalities therefore caused by the construction or operation of this permitted source, nor does it allow the per-
mittee to cause pollution in contravention of Florida Statutes and department rules, except where specifically authorized by an order
from the department granting a variance or exception from department rules or state statutes.
9. This permit is not transferable. Upon sale or legal transfer of the property or facility covered by this permit, the permittee shall
notify the department with in th irty (30) days. The new owner must apply for a permittransfer within thirty (30) days. The permittee
shall be liable for any non-compliance of the permitted soUrce until the transferee applies for and receives a transfer of perrrlit.
10. The permittee, by acceptance of this permit, specifically agrees to allow access to permitted source at reasonable times by de-
partment personnel presenting credentials for the purposes of inspection and testing to determine compliance with this permit and
department rules.
11. This permit does not indicate a waiver of or approval of any other department permit that may be required for other aspects of
the total project.
12. This permit conveys no title to land or water, nor constitutes state recognition or acknowledgement of title,and does not consti-
tute authority for the reclamation of submerged lands unless herein provided and the necessary title or leasehold interests have been
obtained from the state. Only the Trustees of the Internal Improvement Trust Fund may express state opinion as to title.

SPECIFIC CONDITIONS:
1. This permit does not relieve the applicant of the responsibility to
obtain necessary permits, licenses or approvals from other governmental
agencies.
2. The operation of this facility is subject to adherence to all applicable
Department of Environmental Regulation rules.
3. If normal operations are interrupted for a period greater than forty-
eight (48) hours, written notification must be made to the Department
giving the estimated time needed to resume normal operations.
4. No solid wastes shall be left at this facility for more than fourty-
eight (48) hours.

A permit for each Resource Recovery and Management Facility is required. Separate applications for each permit, four copies
each, should be submitted to the District Office of the Department of Environmental Regulation. Complete appropriate sections of
the application for the type of facility proposed: sanitary landfill, incinerator, volume reduction plant, etc.
Each application shall be accompanied by an application fee of $20.00 payable by check drawn in favor of "State of Florida,
Department of Environmental Regulation."
Applicant has the responsibility to provide copies of the application to appropriate city, county and/or regional pollution con-
trol agencies, established pursuant to Section 403.182, Florida Statutes. Applicant shall also clear the application through appropriate
local planning agencies. Comments from any of these agencies shall be forwarded with the application to the Department.
Information contained in the application shall conform to requirements of Chapter 17-7, Florida Administrative Code. All en-
tries should be typed or printed in ink. If additional space is needed, separate, properly identified sheets of paper may be attached.
All documents submitted to support the application should be on 8'h" x 11" paper.
Processing of the application will begin when the foregoing requirements have been met.

The purpose of these instructions is to provide information to those filling out the "Land Disposal Site Data Form" so that the data is
complete and it can be processed without delay. This form is designed for cOmputerization to enable the department to have access to
information on all registered land disposal sites operated in the State of Florida. This data form must be completed by the applicant as
follows:
The control number and the delete, add, change and inactive block are for the department's use only.
1. County - This is self-explanatory.
2. Site- The name of the land disposal site = abbreviate if necessary.
3. Date - The date of this report shall be expressed as the month, day, year, in numbers (Le., 1.1.73).
4. Address - The actual address of the land disposal site or the route by which access to the site is made.
5. Location - The location cif the disposal site in both latitude and longitude, and the section, township and range.
6. Responsible Operating Authority - The authority responsible for the operation of the disposal site.
7. Ownership - The. agency or individual who owns or leases this particular site.
8. Owner Address - The mailing address of the agency shown in Item 7.
9. Owner Phone Number:.... The phone number of the agency shown in Item 7.
10. Population Served - The population which this disposal site is estimated to serve.
11. Number of Acres - The total number of acres of land purchased or leased for the operation of this site.
12. Method of Operation - More than one item may be checked.

37. Liner - Indicate whether an impervious material is being placed on undisturbed earth on which solid waste is deposited to retard
the movement of leachate into the shallow groundwater aquifer.
38. Liner Type - Check one of the materials listed which Is being used for a liner.
39. Well Point System - Indicate whether there is a well point system used to lower the water table in the immediate area of the
landfill.
