FIRST AMENDMENT TO LEASE AGREEMENT TO ADD ROOMS 24 AND 25 -~ ~~ST AMENDMENT TO LEASE AGRJI~MENT RECEIVED AUG 2 2 1994 CITY CLUJ( DEPT. THIS FIRST AMENDMENT TO LEASE AGREEMENT is entered into on / , 1994, by and between the CITY OF CLEARWATER, FLORIDA, a municipal rporat1on of the State of Florida, hereinafter referred to as "LESSOR and David Petrik, DDS, d/b/a Marina Dental & Denture Clinic, P.A., hereinafter referred to as "LESSEE". WHEREAS, the LESSOR and the LESSEE entered into a Lease Agreement on June 1, 1992, wherein the LESSOR leased Room/s 20, 21, 22 and 23 in the Clearwater Marina building to the LESSEE; and WHEREAS, the LESSOR and LESSEE have agreed to amend the Lease Agreement to add room/s 24 and 25 in the Clearwater Marina building, to increase the monthly rental payment; NOW THEREFORE, in consideration of the conditions and mutual covenants contained herein, the parties do hereby agree as follows: 1. In the introductory paragraph of the Lease Agreement, the address for David Petrik, DDS is amended to read: rooms #20, #21, #22, #23, #24 and ~ 25 Causeway Boulevard, Clearwater, Florida 34630. 2. Paragraph 2 is amended to read: a. ~o pay the totul Dum of $43,000.00 for the 60 month term of thiD le~De, ,.hich Dhull be p~id in equal monthly payments of $725.00 $1.150, plus tax. Each monthly payment shall be due and payable on the first day of the month, and shall be delinquent if not paid on or before the fifth day of the month. c. In addition to the first month's rent of $725.00 $1,150.00, which is due and payable on the first day of the lease, the Lessee shall pay $725.00 $375.00 in advance as a deposit to secure the faithful performance of the Lessee's obligations hereunder. The Lessor may deduct from the deposit any amount which might become due from the Lessee to the Lessor for damage to the premises or for any reason or cause whatsoever except rent. At the end of the term of this lease, the deposit amount or the balance thereof, if any, shall be credited to Lessee's last monthly rental payment. 3. Paragraph 6 is amended to read: 6. The Lessee hereby covenants and agrees to pay an additional $50.00 $100.00 a month for electrical service to the premises in addition to the monthly rental. At no expense to the Lessee, the Lessor agrees to furnish a refuse disposal location and a refuse disposal container located outside the demised premises for the use of the Lessee. In the event the city has the rooms metered. the Lessee will pay for electrical service directly to Florida Power Corporation. 4. Exhibit "A" of the Lease Agreement is amended to read: Room/s #20, #21, #22, #23, #24. #25, and Second Floor Closet, Clearwater Marina Building, located on Lots 11 and 12 of City Park Subdivision, according to the map or plat thereof as recorded in Plat Book, page 37 of the public records of Pinellas County, Florida. 5.
