I SERVICE A G R E E MEN T L1 THIS AGREEMENT made and entered into this v9 day of ~./ , 1980, by and between the CITY OF CLEARWATER, FLM.IDA, a ~uniciPal Corporation, llereinaftar referred to as "City", and CHARLES O. DOBBINS, hertilinafter referred to as "Contractor". WITNESSETH: WHEREAS, the City is operating a Rehabilitation Loan Program under the supervision and direction of the Connuunity Development Coordinator who has been designated the Program Director, and WHEREAS, this program is funded solely from funds obtained from the United States Government through the Community Development Block Grant Program and other federal funds obtained by the City, and WHEREAS, the continuation of federal or other sources of funding for this program is speculative, thereby making it expedient to contract for specific rehabilitation services in connection therewith. NOW, THEREFORE, in consideration of the faithful performance by each of the parties of the covenants and agreements made nerein, it is mutually agreed as follows: 1. The City hereby contracts for personal services to be performed under the supervision of the Program Director ane.''fhe Contractor agrees to personally perform those services in accordance with such supervision. 2. The services to be rendered under this agreement shall be those described on Schedule "A", attached hereto and incorporated herein. 3. The term of this agreement shall be four months beginning Bebruary 1, 1980 and ending May 31, 1980. 4. For said services the Contractor shall be paid $4,300.00, payable semi-monthly on the fifteenth and last day of each month during the term hereof. 5. During normal hours of the Conununity Development Office of the City, the City agrees to provide and make available to the Contractor office space, equipment, supplies and telephone service as required by him and to furnish clerical and secretarial servieesfor him at no cost to the Contractor. ;~~~___2._ q " I ~- I 6. The City shall provide such insurance coverage for its benefit and for the benefit of the Contractor as it, in its sole discretion, deems necessary. 7. The Contractor shall not sub-contract any services required hereunder without the prior written consent of the City. 8. This agreement may be terminated by either party at any time upon written notice to the other. I REHABILITATION SPECIALIST SERVICES NATURE OF SERVICE: Professional technical services concerning construction and home repair and remodeling techniques in connection with neighborhood improvement and home rehabilitation programs. SOOPE OF SERVICES: 1. Perform the following technical duties: (a) Schedule and conduct property inspections; (b) Prepare work write-ups (specifications); (c) Maintain property files; (d) Conduct conferences with property owners and contractors; (e) Confer with code inspectors, residents, contractors, architects-, engineers and others regarding codes and construction or rehabilitation plans; (f) Prepare bid documents; (g) Monitor work progress; and (h) Perform perodic and final inspections. 2. Provide technical assistance to homeowner clients. 3. Develop a working relationship with all Contractors. 4. Implement and develop follow-through for established systems, procedures and ferms fer the P~habilitation Loan Program and New C~n8truction or Housing Redevelopment Program. 5. Consult with eligible clients regarding desired rehabilitation activities. 6. Develop and maintain cost of construction information. 7. Assist with work in other areas of the Community Development Block Grant Program as requested. 8. Prepare plans or drawings for construction or rehabilitation work. 9. Maintain written records of services performed and make written reports as requested. 10. Perform related services as requested. Schedule "A"
